摘要
目的 探究白细胞介素(IL)-4、IL-10基因多态性与肺结核患者抗结核药物性肝损伤(ADLI)的关联。方法采用病例对照研究法,选取2018年1月-2020年10月重庆大学附属三峡医院感染科接受标准化治疗的肺结核患者为研究对象,根据ADLI标准判断87例患者为ADLI(ADLI组),按照1∶1比例匹配同期未发生ADLI患者为对照组,提取患者外周血DNA,采用聚合酶链式反应(PCR)检测确定IL-4(rs2243250、rs2070874)、IL-10(rs1800871、rs1800872、rs1800896)基因型,分析IL-4、IL-10基因多态性与肺结核患者发生ADLI的关联。结果ADLI组和对照组IL-4 rs2243250、rs2070874、IL-10 rs1800871、rs1800872基因型分布频数的比较,差异有统计学意义(P<0.05),其中IL-4 rs2243250和rs2070874位点CC基因型、IL-10 rs1800871位点CT基因型、rs1800872位点AC基因型是结核患者发生ADLI的保护因素(P<0.05)。ADLI组IL-4 rs2243250、rs2070874位点T等位基因频数高于对照组,是结核患者发生ADLI的危险因素(P<0.05)。单倍型分析结果显示,ADLI组和对照组IL-4基因T-T、C-C单倍型分布的比较,差异有统计学意义(P<0.05),其中T-T单倍型是结核患者发生ADLI的危险因素,C-C单倍型是保护因素(P<0.05);IL-4基因其他单倍型及IL-10各单倍型与ADLI的发生无明显相关性。结论 IL-4、IL-10基因多态性可能与肺结核患者ADLI易感性有关。
OBJECTIVE To explore the association between interleukin(IL)-4, IL-10 gene polymorphisms and anti-tuberculosis drug-induced liver injury(ADLI) in patients with tuberculosis.METHODS By means of case-control study, the patients with tuberculosis who received standardized therapy in infection control department of Three Gorges Hospital Affiliated to Chongqing University from Jan 2018 to Oct 2020 were recruited as the study objects, 87 patients who were diagnosed with ADLI based on the criteria for diagnosis of ADLI were assigned as the ADLI group, meanwhile, the patients who did not have ADLI were chosen as the control group in a 1∶1 ratio. The peripheral blood DNA were extracted from the patients, the genotypes of IL-4(rs2243250, rs2070874) and IL-10(rs1800871, rs1800872, rs1800896)were determined by polymerase chain reaction(PCR), and the association between the IL-4, IL-10 gene polymorphisms and the incidence of ADLI in the tuberculosis patients was observed.RESULTS There were significant differences in the frequencies of genotypes at rs2243250 and rs2070874 of IL-4 and rs1800871, rs1800872 of IL-10 between the ADLI group and the control group(P<0.05), the CC genotype at rs2243250 and rs2070874 loci of IL-4, CT genotype at rs1800871 locus of IL-10 and AC genotype at rs1800872 locus were protective factors for ADLI(P<0.05). The frequencies of T allele at rs2243250 and rs2070874 loci of IL-4 were significantly higher in the ADLI group than in the control group, which are the risk factors for ADLI in the tuberculosis patients(P<0.05). The result of haplotype analysis showed that there were significant differences in the T-T and C-C haplotypes of IL-4 gene between the ADLI group and the control group(P<0.05), T-T haplotype was the risk factor for ADLI in the tuberculosis patients, and the C-C haplotype was the protective factor(P<0.05);the other haplotypes of IL-4 genes and the haplotypes of IL-10 were not significantly associated with the incidence of ADLI.
