摘要
目的:探讨暖宫方联合乌鸡白凤丸治疗寒实证月经不调的临床疗效,为临床治疗寒实证月经不调提供参考。方法:随机选取随州市广水市第二人民医院2016年9月-2019年8月收治的寒实证月经不调患者160例,随机分为两组。其中观察组80例采用暖宫方联合乌鸡白凤丸法治疗,对照组80例给予黄体酮软胶囊和戊酸雌二醇片口服进行临床治疗。观察比较两组患者的治疗效果,记录并分析两组患者在常规妇科体检检查中的血流动力学相关指标以及性激素水平。结果:观察组患者治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。经治疗,两组患者卵巢内血流动力学参数收缩期峰值流速(Peak Systolic Velocity,PSV)与治疗前相比均显著升高(P<0.05),搏动指数(Pulsatility Index,PI)和阻力指数(Resistance Index,RI)水平均显著降低(P<0.05)。治疗后两组患者PSV和PI水平接近,差异无统计学意义(P>0.05),而观察组RI水平低于对照组(P<0.05)。两组患者血清中雌二醇(Estradiol,E2)、促黄体生成素(Luteinizing Hormone,LH)和卵泡刺激素(Folliclestimulating Hormone,FSH)水平较治疗前均显著上升(P<0.05),且治疗后观察组相关激素水平高于对照组(P<0.05)。结论:暖宫方联合乌鸡白凤丸用于寒实证月经不调患者具有良好疗效,且能改善患者性激素以及血流动力学相关指标水平,此种治疗方法值得在临床推广。
Objective:Clinical efficacy of the Nuangong prescription(暖宫方)plus Wuji Baifeng Wan(乌鸡白凤丸)on irregular menstruation with coldness-excess syndrome was explored to provide more references for clinical treatment of the disease.Methods:A total of 160 cases from September 2016 to August 2019 were randomly selected and divided into two groups.Among them,the 80 cases in the observation group were treated with the Nuangong prescription plus Wuji Baifeng Wan;and 80 cases in the control group were given progesterone soft capsules plus estradiol valerate tablets.Results:The total efficiency in the observation group was higher than the the control group,and the difference was statistically significant(P<0.05).After treatment,the intraovarian hemodynamic parameters PSV of patients in the two groups were both significantly increased compared with before treatment(P<0.05),and the levels of PI and RI were significantly decreased(P<0.05).After treatment,the PSV and PI levels of patients in the two groups were similar,and there was no statistical difference(P>0.05);while the RI level in the observation group was lower than the control group(P<0.05).After treatment,the levels of E2,LH and FSH in serum of patients in the two groups were obviously higher than those before treatment(P<0.05);and the levels of related hormones in the observation group were higher than the control group(P<0.05).Conclusion:The Nuangong prescription plus Wuji Baifeng Wan shows a good effect on irregular menstruation with coldness-excess syndrome,can improve the levels of sex hormones and hemodynamic related indicators.This treatment method is worthy of clinical promotion.
