血液透析配合HP方案对慢性肾衰竭患者钙磷代谢及炎性因子表达的影响 被引量:3

Effects of Hemodialysis Combined with HP Regimen on Calcium and Phos⁃phorus Metabolism and Expression of Inflammatory Factors in Patients with Chronic Renal Failure

摘要 目的探讨血液透析联合血液灌流(hemoperfusion,HP)方案在慢性肾衰竭治疗中的作用。方法选取2020年1月-2021年11月南京银城康复医院收治的慢性肾衰竭患者60例进行研究,根据随机数表法均分两组,予以对照组30例患者血液透析,观察组30例患者在上述基础上与HP方案联合应用,对比两组毒素清除效果、钙磷代谢情况、炎症因子水平和营养状态。结果治疗前,两组各项指标组间对比,差异无统计学意义(P>0.05);治疗后,观察组血肌酐为(502.18±72.56)μmol/L,血尿素氮水平为(12.72±1.54)mmol/L,β_(2)-微球蛋白水平为(18.64±3.72)mg/L,血磷为(4.21±0.32)mg/dl,钙磷乘积为(38.24±4.12)mg^(2)/dl^(2),C反应蛋白为(8.72±1.32)mg/L,肿瘤坏死因子-α为(8.34±1.18)pg/mL,降钙素原为(6.98±1.16)ng/mL,血钙为(8.72±1.18)mg/dl,血红蛋白为(90.21±5.27)g/L,转铁蛋白为(2.32±0.27)g/L,总蛋白为(70.51±4.18)g/L,白蛋白为(33.12±1.72)g/L均优于对照组,差异有统计学意义(t=5.421、5.171、3.769、21.902、6.063、7.530、10.624、4.509、2.970、4.040、5.155、7.762、5.888,P<0.05)。结论在慢性肾衰竭患者的治疗中联合应用血液透析和HP方案,可获得理想的毒素清除效果,对钙磷代谢和营养状况有明显的改善作用,也可减少炎症因子释放,存在推广价值。 Objective To investigate the effect of hemodialysis combined with hemoperfusion(HP)in the treatment of chronic renal failure.Methods A total of 60 patients with chronic renal failure admitted to Nanjing Yincheng Reha⁃bilitation Hospital from January 2020 to November 2021 were selected for this study.According to the random num⁃ber table method,they were divided into two groups,30 patients in the control group were treated with hemodialysis,and 30 patients in the observation group were combined with HP regimen.The toxin removal effect,calcium and phosphorus metabolism,inflammatory factor levels and nutritional status of the two groups were compared.Results Before treatment,there was no statistically significant difference in each indicator between the two groups(P>0.05).After treatment,serum creatinine was(502.18±72.56)μmol/L,blood urea nitrogen was(12.72±1.54)mmol/L,β_(2)-mi⁃croglobulin was(18.64±3.72)mg/L,blood phosphorus was(4.21±0.32)mg/dl,and calcium phosphorus product was(38.24±4.12)mg^(2)/dl^(2),C-reactive protein were(8.72±1.32)mg/L,tumor necrosis factor-αwere(8.34±1.18)pg/mL and procalcitonin were(6.98±1.16)ng/mL,serum calcium(8.72±1.18)mg/dl,hemoglobin(90.21±5.27)g/L,trans⁃ferrin(2.32±0.27)g/L,total protein(70.51±4.18)g/L,albumin(33.12±1.72)g/L were all better than those in the con⁃trol group,and the difference was statistically significant(t=5.421,5.171,3.769,21.902,6.063,7.530,10.624,4.509,2.970,4.040,5.115,7.762,5.888,P<0.05).Conclusion Combined application of hemodialysis and HP regi⁃men in the treatment of patients with chronic renal failure can achieve ideal toxin removal effect,and can significantly improve calcium and phosphorus metabolism and nutritional status.It can also reduce the release of inflamma⁃tory factors and has promotion value.
