摘要
目的:探讨基于Teach-back理论的健康教育对接受体外受精-胚胎移植(IVF-ET)且反复种植失败(RIF)患者疾病感知控制、自我效能及治疗依从性的影响。方法:选取2018年4月1日~2020年11月1日进行诊疗的接受IVF-ET且RIF次数≥2次的患者267例作为研究对象。按随机数字表法将患者分为研究组135例和对照组132例,对照组采用常规健康教育,研究组采用基于Teach-back理论的健康教育;比较两组干预前后疾病感知控制能力[采用简易版疾病感知问卷(B-IPQ)]和自我效能感[采用一般自我效能感量表(GSES)]及干预后治疗依从性和生育生活质量[采用生育生活质量问卷(FertiQoL)]。结果:干预后,两组B-IPQ中周期性、个人控制、治疗控制、情感控制、疾病一致性和对症状的识别评分均高于干预前(P<0.05),且研究组高于对照组(P<0.01);干预后,两组GSES评分均高于干预前(P<0.01),且研究组高于对照组(P<0.01);干预后,研究组依从性高于对照组(P<0.05),FertiQoL中情感、精神/躯体、夫妻关系及社会关系领域评分均高于对照组(P<0.01)。结论:基于Teach-back理论的健康教育有助于提高IVF-ET且RIF患者个人、情感、疾病识别等疾病感知控制能力和自我效能感,并有助于提高患者治疗依从性,改善生育生活质量,可在临床推广应用。
Objective:To explore the effect of health education based on teach-back theory on disease perception control,self-efficacy and treatment compliance in patients with repeated implantation failure(RIF).Methods:A total of 267 patients who underwent in vitro fertilization-embryo transfer(IVF-ET)and RIF is equal or greater than twice,and admitted during April 1,2018 to November 1,2020 were selected as the research subjects.According to the random number table method,the patients were divided into the study group(135 cases)and the control group(132 cases).The control group was given routine health education,and on this basis,the study group was given health education based on the teach-back theory.Disease perception control by B-IPQ,self-efficacy by GSES,treatment compliance and quality of reproductive life after intervention by the Fertility Quality of Life Questionnaire(FertiQoL)between the two groups were compared before and after intervention.Results:After intervention,the scores of periodicity,personal control,treatment control,emotional control,disease consistency,and symptom recognition were higher in both groups than those before intervention(P<0.05),and those in the study group were higher than those in the control group(P<0.01).After intervention,the GSES scores were higher in both groups than those before intervention(P<0.05),and those in the study group were higher than those in the control group(P<0.01).After intervention,the compliance rate in the study group was higher than that in the control group(P<0.05).After intervention,the emotional,mental/physical,spousal relationship and social relation scores by FertiQoL in the study group were all higher than those in the control group(P<0.01).Conclusion:Health education based on the teach-back theory can help to improve the disease perception control capabilities and self-efficacy such as personal,emotional,and disease recognition in patients with IVF-ET and RIF,help to enhance their treatment compliance,and improve the quality of reproductive life,which is w
