摘要
目的:探索瑞舒伐他汀对老年轻度认知功能障碍的影响以及与载脂蛋白E(APOE)基因多态性的关系.方法:选取2020年1月~2021年9月在我院就诊治疗的轻度认知功能障碍老年患者220例.采用随机数字表法将患者分为对照组和观察组,每组110例.观察组采用PCR-RFLP法检测ApoE基因,根据ApoE基因分为ApoEε4(-)组和ApoEε4(+)组.对照组使用多奈哌齐片治疗;观察组使用多奈哌齐片和瑞舒伐他汀钙片治疗,两组都治疗6个月.观察两组患者临床疗效、美国国立卫生研究院简易精神状态量表(MMSE)评分、日常生活能力量表(ADL)评分、总胆固醇(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)水平和IL-1β和IL-6水平和药物不良反应.结果:观察组的临床总有效率显著高于对照组;观察组患者中ApoE基因型占比最高是ε3/3纯合子(49.09%),最低的是ε2/2纯合子(2.72%),ε4携带者的比例为37.27%.治疗后,与对照组相比,ApoEε4(+)和ApoEε4(-)组MMSE分数和HDL水平更高,ADL分数、TC、LDL、IL-1β和IL-6水平更低;与ApoEε4(+)组相比,ApoEε4(-)组MMSE分数和HDL水平更高,ADL分数、TC、LDL、IL-1β和IL-6水平更低.结论:瑞舒伐他汀可以改善老年轻度认知功能障碍患者认知功能和生活能力,降低血脂水平和炎性水平,并且与ApoE基因多态性相关.
Objective To explore the effect of rosuvastatin on mild cognitive impairment in the elderly and its relationship with Apolipoprotein E(ApoE)gene polymorphism.Methods 220 elderly patients with mild cognitive impairment treated in our hospital from January 2020 to September 2021 were selected.The patients were randomly divided into control group and ob-servation group,with 110 cases in each group.The observation group detected ApoE gene by PCR-RFLP,and was divided into ApoE according to ApoE geneεGroup 4(-)and ApoEε4(+)group.The control group was treated with donepezil tablets;The observation group was treated with donepezil tablets and rosuvastatin calcium tablets.The clinical efficacy,MMSE score,ADL score,total cholesterol(TC),high density lipoprotein(HDL),low density lipoprotein(LDL),IL-1β、IL-6 level and adverse drug reactions were observed.Results This study found that the total clinical effective rate of the observation group was significantly higher than that of the control group.The proportion of ApoE genotype in the observation group was the highestε3/3 ho-mozygous(49.09%),the lowest isε2/2 homozygous(2.72%),The proportion ofε4 carriers was 37.27%.After treatment,com-pared with the control group,ApoEε4(+)and ApoEε4(-)group had higher MMSE scores and HDL levels,lower ADL scores,TC,LDL,IL-1βand IL-6 levels,and ApoEε4(+)group compared with ApoEε4(-)group had higher MMSE scores and HDL levels,and lower ADL scores,TC,LDL,IL-1βand IL-6 level.Conclusion Rosuvastatin can improve cognitive function and living ability of elderly patients with mild cognitive impairment,reduce blood lipid level and inflammatory level,and is related to ApoE gene polymorphism.
