摘要
目的探讨不同时机行动脉瘤夹闭术治疗颅内破裂动脉瘤的效果。方法抽取2017年1月至2019年12月济源市人民医院收治的颅内破裂动脉瘤患者63例,均行开颅动脉瘤夹闭术,根据治疗时机将其分为早期组(发病24 h内手术,23例)、中期组(发病24~72 h手术,20例)、晚期组(发病72 h后手术,20例)。比较三组预后良好率及并发症发生率。结果预后良好率从高到低依次为早期组、中期组、晚期组,且早期组(91.30%,21/23)高于晚期组(70.00%,14/20),P<0.05。三组术中出血、脑血管痉挛、术后动脉瘤再出血、脑积水等并发症发生率比较,差异未见统计学意义(P>0.05)。结论颅内破裂动脉瘤予以开颅动脉瘤夹闭治疗可获得较好的效果,手术时机越早预后越好,且不会增加并发症,安全性好。
Objective To investigate the effect of craniotomy aneurysm clipping in different times in the treatment of ruptured intracranial aneurysms.Methods Sixty-three patients with intracranial ruptured aneurysm who were admitted to Jiyuan People’s Hospital from January 2017 to December 2019 were selected,and all selected patients underwent craniotomy aneurysm clipping.According to the timings of treatment,they were divided into early group(within 24 hours after onset,23 cases),intermediate group(24-72 hours after onset,20 cases),and advanced group(>72 hours after onset,20 cases).The good prognosis rate and complication rate were compared among the three groups.Results The good prognosis rate of the early group was the highest,followed by intermediate group and advanced group in sequence,and good prognosis rate of the early group(91.30%,21/23)was higher than that of the advanced group(70.00%,14/20),P<0.05.There was no significant difference in incidence of intraoperative hemorrhage,cerebral vasospasm,postoperative aneurysm rebleeding,and hydrocephalus among the three groups(P>0.05).Conclusions The craniotomy aneurysm clipping in the treatment of ruptured intracranial aneurysms can achieve better outcomes,and performing operation earlier can get better prognosis,and will not increase complication,with good safety.
