3D打印技术在颅内动脉瘤诊断及治疗中的应用价值 被引量:2

摘要 目的探讨3D打印技术在颅内动脉瘤诊断及治疗中的应用价值。方法选取贵港市人民医院2018年2月至2020年12月收治的50例疑似颅内动脉瘤患者作为研究对象,所有患者均接受3D打印技术及3D-DSA检查。以3D-DSA检查结果作为金标准,分析3D打印技术对颅内动脉瘤诊断的敏感性、特异性、阳性预测值、阴性预测值;记录确诊颅内动脉瘤患者在3D打印技术辅助下手术治疗情况。结果 50例疑似颅内动脉瘤患者,经3D-DSA检查确诊颅内动脉瘤40例,非颅内动脉瘤10例;3D打印技术检查诊断颅内动脉瘤38例,非颅内动脉瘤12例。以3D-DSA检查结果作为金标准,3D打印技术对颅内动脉瘤诊断敏感性为92.50%、特异性为90.00%、阳性预测值为97.37%、阴性预测值为75.00%。确诊40例颅内动脉瘤患者中,行开颅动脉瘤夹闭术10例(25.00%)、动脉瘤介入栓塞术30例(75.00%),40例患者手术治疗有效率为97.50%、手术致残率为0.00%(0/40)、手术致死率为0.00%(0/40)。结论 3D打印技术对颅内动脉瘤诊断的敏感性及特异性均较高。 Objective To investigaate the practical value of 3 D printing technology in the diagnosis and treatment of intracranial aneurysms. Methods Fifty patients with suspected intracranial aneurysms admitted to Guigang Peoples’ Hospital from February 2018 to December 2020 were selected as the subjects. All patients received 3 D printing technology and 3 D-DSA examination. Using 3 D-DSA examination results as the gold standard,the sensitivity,specificity,the positive predictive value and the negative predictive value of 3 D printing technology for the diagnosis of intracranial aneurysms were analyzed. The surgical treatment of patients with confirmed intracranial aneurysms assisted by 3 D printing technology were recorded.Results Among the 50 patients with suspected intracranial aneurysms,40 cases were diagnosed as intracranial aneurysms and 10 cases were non intracranial aneurysms by 3 D-DSA;38 cases of intracranial aneurysms and 12 cases of non intracranial aneurysms were diagnosed by 3 D printing technology. Taking the results of 3 D-DSA as the gold standard,the sensitivity,specificity,positive predictive value and negative predictive value of 3 D printing in the diagnosis of intracranial aneurysms were 92. 50%,90. 00%,97. 37%and 75. 00%,respectively. Among the 40 patients with intracranial aneurysms,10 patients( 25. 00%)underwent craniotomy aneurysm clipping and 30 patients( 75. 00%) underwent aneurysm interventional embolization. The effective rate of surgical treatment,surgical disability rate and operative mortality rate of 40 patients were 97. 50%( 39/40),0. 00%( 0/40) and 0. 00( 0/40). Conclusions 3 D printing technology has high sensitivity and specificity for the diagnosis of intracranial aneurysms.
