摘要
目的分析颅内动脉瘤破裂早期行开颅动脉瘤夹闭的临床效果及预后。方法100例颅内动脉瘤破裂早期(发病时间均<3 d)患者,均采用开颅动脉瘤夹闭治疗。分析不同Hunt-Hess分级患者的相关指标、预后情况,不同动脉瘤大小和位置患者夹闭程度。结果100例患者均手术成功,平均手术时间为(141.40±27.62)min,平均术中出血量为(71.62±20.21)ml,平均住院时间为(30.23±8.30)d;14例患者出现并发症,并发症发生率为14.00%。100例患者中,66例(66.00%)患者预后良好,34例(34.00%)患者预后差。预后差患者中包括:轻度残疾13例(13.00%)、中度致残7例(7.00%)、重度致残5例(5.00%)、植物生存6例(6.00%)、死亡3例(3.00%)。巨型及大型动脉瘤患者的夹闭程度比小型及一般动脉瘤患者差;不同动脉瘤位置患者的夹闭程度差异不大。结论采用开颅动脉瘤夹闭对颅内动脉瘤破裂早期患者进行治疗效果确切,可将并发症和死亡率降低,安全性良好,患者无不良预后。
Objective To analyze the clinical effect and prognosis of craniotomy aneurysm clipping at early stage in ruptured intracranial aneurysms.Methods 100 patients with ruptured intracranial aneurysms at early stage(all<3 d of onset)were treated with craniotomy aneurysm clipping.The relevant indexes and prognosis of patients with different Hunt-Hess grades,and the degree of clamping in patients with different aneurysm sizes and locations were analyzed.Results All 100 patients were operated successfully with a mean operative time of(141.40±27.62)min,mean intraoperative blood loss of(71.62±20.21)ml,mean hospital stay of(30.23±8.30)d,and complications in 14 patients,with a complication rate of 14.00%.Among the 100 patients,66 patients(66.00%)had good prognosis and 34 patients(34.00%)had poor prognosis.The patients with poor prognosis included:13 cases(13.00%)of mild disability,7 cases(7.00%)of moderate disability,5 cases(5.00%)of severe disability,6 cases(6.00%)of plant survival,and 3 cases(3.00%)of death.The degree of clamping was worse in patients with giant and large aneurysms than in patients with small and average aneurysms;there was little difference in the degree of clamping in patients with different aneurysm locations.Conclusion Craniotomy aneurysm clipping is effective in the treatment of patients at the early stage of ruptured intracranial aneurysms,and can reduce complications and mortality,with high safety and no adverse prognosis.
