摘要
目的探讨高压氧(HBO)在脑肿瘤和脑动脉瘤术后康复中的临床效果。方法300例脑肿瘤和脑动脉瘤术后患者,根据治疗方法不同分为实验组与对照组,各150例。对照组接受常规药物治疗,实验组在对照组基础上采用HBO治疗。比较两组治疗前后的神经功能缺损程度量表(NFD)评分和Barthel指数、大脑中动脉流速、大脑动脉脑血管痉挛(CVS)情况。结果治疗前,两组NFD评分和Barthel指数比较,差异无统计学意义(P>0.05);治疗后,实验组NFD评分(7.3±5.4)分低于对照组的(10.4±4.1)分,Barthel指数(49.83±13.51)分高于对照组的(36.51±10.48)分,差异均有统计学意义(P<0.05)。治疗前,两组大脑中动脉流速、大脑动脉CVS占比比较,差异无统计学意义(P>0.05);治疗后,实验组大脑中动脉流速(97.8±22.4)cm/s慢于对照组的(120.3±19.9)cm/s,大脑动脉CVS占比14.0%低于对照组的32.0%,差异均有统计学意义(P<0.05)。结论HBO疗法对脑肿瘤和脑动脉瘤术后康复的效果较好,可改善患者的神经功能缺损情况,提高生活质量,具有较高的临床价值。
Objective To discuss the clinical effect of hyperbaric oxygen(HBO)on postoperative rehabilitation of brain tumors and cerebral aneurysms.Methods A total of 300 patients with brain tumor and cerebral aneurysm after surgery were divided into experimental group and control group according to different treatment methods,with 150 cases in each group.The control group received conventional drug treatment,and the experimental group received HBO treatment on the basis of the control group.The neurological function deficit(NFD)score,Barthel index,middle cerebral artery flow velocity,and cerebral artery cerebral vasospasm(CVS)before and after treatment were compared between the two groups.Results Before treatment,there was no statistically significant difference in NFD score and Barthel index between the two groups(P>0.05).After treatment,the NFD score(7.3±5.4)points of the experimental group was lower than(10.4±4.1)points of the control group,and the Barthel index(49.83±13.51)points was higher than(36.51±10.48)points of the control group.All the differences were statistically significant(P<0.05).Before treatment,there was no statistically significant difference in the middle cerebral artery flow rate and the proportion of cerebral artery CVS between the two groups(P>0.05).After treatment,the middle cerebral artery flow rate(97.8±22.4)cm/s in the experimental group was slower than(120.3±19.9)cm/s in the control group,and the proportion of cerebral artery CVS 14.0%was lower than 32.0%in the control group.All the differences were statistically significant(P<0.05).Conclusion HBO therapy is of high clinical value for postoperative rehabilitation of brain tumors and cerebral aneurysms,and can improve the quality of life of patients.
