摘要
目的探究不同牙种植术对前牙列缺失患者牙种植修复效果及龈沟液炎症因子水平的影响。方法选取2019年3月至2020年3月收治的78例前牙列缺失患者,按照抓阄法随机分为甲组(38例,常规牙种植术)和乙组[40例,借助计算机辅助设计与制造(CAD/CAM)牙种植导板进行牙种植术]。比较两组患者术后种植体的三维位置偏差、语言功能评分、咀嚼效率、牙周环境指标及龈沟液炎症因子水平。结果乙组种植体角度、深度、根部、头部偏差均小于甲组(P<0.05)。术后30 d,两组的语言功能评分和咀嚼效率均升高,且乙组明显高于甲组(P<0.05)。术后30 d,两组的软垢指数、龈沟出血指数、菌斑指数、肿瘤坏死因子-α(TNF-α)、单核细胞趋化蛋白-1(MCP-1)水平均降低,且乙组明显低于甲组(P<0.05)。结论CAD/CAM牙种植导板在前牙列缺失患者牙种植修复中的效果显著,可显著提高种植精确度,促进患者语言功能和咀嚼功能提高,改善牙周环境,降低龈沟液炎症因子的表达。
Objective To explore the influences of different dental implants on dental implant restoration effect and gingival crevicular fluid inflammatory factors levels in patients with anterior dentition loss.Methods A total of 78 patients with anterior dentition loss admitted from March 2019 to March 2020 were selected and randomly divided into group A(38 cases,conventional dental implant)and group B[40 cases,with the help of computer aided design/computer aided manufacturing(CAD/CAM)dental implant guide plate for dental implant]according to the lottery method.The three-dimensional position deviation,language function score,masticatory efficiency,periodontal environment indexes and gingival crevicular fluid inflammatory factors levels were compared between the two groups.Results The angle,depth,root and head deviation of implants in the group B was less than those in the group A(P<0.05).At 30 d after operation,the language function score and masticatory efficiency in both groups increased,and those in the group B was significantly higher than the group A(P<0.05).At 30 d after operation,the debris index,gingival crevicular bleeding index,plaque index,tumor necrosis factor-α(TNF-α),and monocyte chemotactic protein-1(MCP-1)levels in both groups decreased,and those in the group B was significantly lower than the group A(P<0.05).Conclusion CAD/CAM dental implant guide plate has a significant effect in the dental implant restoration of patients with anterior dentition loss,it can significantly improve the implant accuracy,promote the improvement of patients'language function and masticatory function,improve the periodontal environment,and reduce the expression of gingival crevicular fluid inflammatory factors.
