摘要
目的:研究口腔正畸与种植义齿联合治疗成人前牙错牙合畸形及牙列缺损的疗效及对患者牙齿功能、美观效果和生活质量的影响。方法:回顾性分析2016年4月~2019年4月我院94例前牙牙列缺失伴错牙合畸形患者临床资料,并根据治疗方案不同分观察组53例和对照组41例,其中观察组实施口腔正畸再进行种植修复治疗,对照组仅给予种植义齿修复。比较两组治疗效果并随访术后6个月时患者牙齿功能、美观效果和生活质量。结果:观察组和对照组种植修复合格率分别为96.23%和82.93%(P<0.05),两组种植修复效果差异有统计学意义(P<0.05)。术后6个月两组咀嚼功能、吞咽功能和发音功能评分明显升高(P<0.05),且观察组咀嚼和发音功能评分高于对照组,差异有统计学意义(P<0.05)。观察组术后前牙排列整齐、覆盖完整及咬合正常患者所占比例明显高于对照组(P<0.05)。术后6个月两组OHIP-I量表功能限制、生理疼痛与不适、心理与社交评分及总分均明显降低(P<0.05),且观察组各领域评分及总分均低于对照组,差异有统计学意义(P<0.05)。结论:口腔正畸与种植义齿联合治疗成人前牙错牙合畸形可提升种植修复合格率,改善牙齿功能和美观效果,提升患者生活质量。
Objective:To study the efficacy of orthodontics combined with implant denture in the treatment of adult anterior teeth malocclusion and dentition defect and its effects on dental function,aesthetic effects and quality of life of patients.Methods:A retrospective analysis was performed on 94 patients with anterior dentition defect and malocclusion in our hospital from April 2016 to April 2019.According to the treatment regimens,the patients were divided into observation group(53 cases)and control group(41 cases).Observation group was given orthodontics and then given implant restoration while control group was only treated with implant denture restoration.The treatment effects were compared between the two groups,and the dental function,aesthetic effects and quality of life were followed up at 6 months after surgery.Results:The qualified rates of implant restoration in observation group and control group were 96.23% and 82.93% respectively(P<0.05),and there were significant differences in the effects of implant restoration between the two groups(P<0.05).At 6 months after surgery,the scores of chewing function,swallowing function and phonic function were significantly increased in the two groups(P<0.05),and the scores of chewing function and phonic function in observation group were higher than those in control group(P<0.05).The proportions of patients with neatly arranged anterior teeth,intact covering and normal occlusion in observation group were significantly higher than those in control group(P<0.05).The scores of functional limitation,physiological pain and discomfort and psychology and social contact and total score of OHIP-I scale were significantly decreased at 6 months after surgery(P<0.05),and the scores of fields and total score in observation group were lower than those in control group(P<0.05).Conclusion:The combination of orthodontics and implant denture for adult anterior teeth malocclusion can improve the qualified rate of implant restoration,promote dental function and aesthetic effects,and enhance
