摘要
目的 探究在糖尿病合并下肢骨折老年患者中应用围术期血糖控制方法的临床效果。方法 以该院2018年2月—2021年5月收治的80例糖尿病合并下肢骨折的老年患者为研究对象,采用随机法分为两组,各40例。对照组给予常规干预,研究组在对照组基础上增加围术期血糖控制。对两组患者的血糖控制情况、并发症发生率和住院时间、骨折愈合时间等情况进行比较。结果 治疗前,两组患者的血糖水平差异无统计学意义(P>0.05);治疗后,研究组患者的空腹血糖和餐后2 h血糖值分别为(5.45±0.33)mmol/L和(6.34±0.54)mmol/L,低于对照组,差异有统计学意义(t=11.450、13.572,P<0.05)。两组患者均发生并发症,研究组患者的并发症发生率为10.00%,低于对照组的37.50%,差异有统计学意义(χ^(2)=8.352,P<0.05)。研究组患者的切口愈合时间和骨折愈合时间分别为(12.87±1.76)、(92.67±6.99)d,短于对照组,差异有统计学意义(t=10.735、9.271,P<0.05)。结论 在糖尿病合并下肢骨折的老年患者治疗中加强围术期血糖控制,其效果显著,有利于促进患者康复,实现对患者血糖水平的控制,减少患者并发症发生率,意义显著。
Objective To explore the clinical effect of perioperative blood glucose control in elderly patients with diabetes mellitus complicated with lower extremity fractures.Methods A total of 80 elderly patients with diabetes mellitus and lower extremity fractures admitted to the hospital from February 2018 to May 2021 were selected as the research objects and randomly divided into two groups,40 cases in each.The control group was given routine intervention,and the study group increased perioperative blood glucose control on the basis of the control group.The blood glucose control,complication rate,hospital stay and fracture healing time were compared between the two groups.Results Before treatment,there was no significant difference in blood glucose levels between the two groups(P >0.05);after treatment,the fasting blood glucose and 2 h postprandial blood glucose values of the study group were(5.45±0.33) mmol/L and(6.34±0.54) mmol/L,which was lower than the control group,and the difference was statistically significant(t =11.450,13.572,P <0.05).Complication occurred in both groups,and the incidence of complication in the study group was 10.00%,which was lower than 37.50% in the control group,and the difference was statistically significant(χ~2=8.352,P<0.05).The incision healing time and fracture healing time of the patients in the study group were(12.87±1.76) d and(92.67±6.99) d,respectively,shorter than those in the control group,and the difference was statistically significant(t=10.735,9.271,P<0.05).Conclusion Strengthening perioperative blood glucose control in the treatment of elderly patients with diabetes and lower extremity fractures has a significant effect,which is beneficial to promote the recovery of patients,control the blood glucose level of patients,and reduce the incidence of complication in patients,which is of significant significance.
