摘要
目的研究对老年糖尿病患者合并髋部骨折在围手术期使用胰岛素泵治疗的效果。方法抽取该院2017年7月—2020年6月收治的72例糖尿病髋部骨折患者按照入院时间分成两组,给予前36例患者围手术期应用皮下注射胰岛素进行治疗并设为常规组,给予后36例患者围手术期间用胰岛素泵进行持续皮下注射并设为研究组,观察两组患者相关指标情况。结果术前两组空腹血糖及餐后2 h血糖差异无统计学意义(P>0.05);术后研究组空腹血糖(6.02±0.24)mmol/L、餐后2 h血糖(6.78±0.26)mmol/L优于常规组的(7.11±0.16)mmol/L、(8.15±0.27)mmol/L,差异有统计学意义(t=22.673、21.930,P<0.05);研究组血糖达标时间(2.88±1.12)d、胰岛素用量(28.19±4.80)U/d,明显低于常规组的(4.12±1.31)d、(37.23±5.30)U/d,差异有统计学意义(t=4.317、7.585,P<0.05);研究组骨钙素为(7.78±3.21)μg/L,明显高于常规组的(6.12±3.02)μg/L,差异有统计学意义(t=2.260,P<0.05)。结论对老年糖尿病合并髋部骨折患者在接受胰岛素泵的围手术期治疗后,可提高治疗效果,还对糖尿病病情控制起到辅助作用。
Objective To study the effect of insulin pump treatment in elderly diabetic patients with hip fracture during perioperative period.Methods A total of 72 diabetic hip fracture patients admitted to the hospital from July 2017 to June 2020 were divided into two groups according to the time of admission.The first 36 patients were treated with subcutaneous insulin injection during the perioperative period and set as the routine group.The last 36 patients were given continuous subcutaneous injections with insulin pumps during the perioperative period and were set as the study group to observe the relevant indicators of the two groups.Results There was not statistically significant difference in fasting blood glucose and 2 h postprandial blood glucose between the two groups before operation(P>0.05);postoperative fasting blood glucose(6.02±0.24)mmol/L and 2 h postprandial blood glucose(6.78±0.26)mmol/L was better than the conventional group(7.11±0.16)mmol/L,(8.15±0.27)mmol/L,the difference was statistically significant(t=22.673,21.930,P<0.05);the study group blood glucose standard time(2.88±1.12)d,The insulin dosage(28.19±4.80)U/d was significantly lower than the(4.12±1.31)d and(37.23±5.30)U/d of the routine group,the difference was statistically significant(t=4.317,7.585,P<0.05);The osteocalcin of the study group was(7.78±3.21)μg/L which was significantly higher than the(6.12±3.02)μg/L of the routine group,and the difference was statistically significant(t=2.260,P<0.05).Conclusion After receiving perioperative treatment of insulin pump for elderly diabetic patients with hip fracture,the treatment effect can be improved,and it can also assist in the control of diabetes.
