摘要
目的:研究四维超声时间-空间关联成像(STIC)联合高分辨率血流成像(HDF)在胎儿心脏瓣膜病筛查中的应用价值。方法:选择疑似胎儿心脏瓣膜病的孕妇74例为研究对象,均行二维超声检查、STIC联合HDF检查。比较两种检查方法单胎胎儿筛选时间和图像采集时间,以及准确率、灵敏度、特异度、阳性预测值及阴性预测值、切面满意度。结果:与二维超声检查比较,STIC联合HDF检查单胎胎儿筛选时间和图像采集时间较低(均P<0.05)。与二维超声检查比较,STIC联合HDF检查准确率较高(P<0.05)。与二维超声检查比较,STIC联合HDF检查灵敏度、特异度、阳性预测值及阴性预测值较高(均P<0.05)。STIC联合HDF检查切面满意度为98.6%(73/74),二维超声检查切面满意度为93.2%(69/74),两者比较差异无统计学意义(P>0.05)。结论:STIC联合HDF检查用于胎儿心脏瓣膜病筛查,可尽早明确疾病,提高诊断效能。
Objective:To study the the application value of spatio-temporal image correlation(STIC)combined with high definition flow(HDF)in fetal valvular heart disease screening.Methods:A total of 74 pregnant women with suspected fetal valvular heart disease were selected as the study subjects.All patients underwent two-dimensional ultrasound examination,STIC combined with HDF examination.The screening time and image acquisition time of singleton fetuses were compared between wo methods,as well as the accuracy,sensitivity,specificity,positive and negative predictive values,and section satisfaction.Results:Compared with two-dimensional ultrasound examination,STIC combined with HDF examination showed lower screening time and image acquisition time for singleton fetuses(all P<0.05).Compared with two-dimensional ultrasound examination,STIC combined with HDF examination had higher accuracy(P<0.05).Compared with two-dimensional ultrasound examination,STIC combined with HDF examination had higher sensitivity,specificity,positive predictive value and negative predictive value(all P<0.05).The degree of section satisfaction was 98.6%(73/74)for STIC combined with HDF examination and 93.2%(69/74)for two-dimensional ultrasound examination,and there was no significant difference between two methods(P>0.05).Conclusion:STIC combined with HDF examination for fetal valvular heart disease screening can identify the disease as early as possible,and improve the diagnostic efficiency.
