二维超声三切面法筛查胎儿先天性心脏畸形的可行性及局限性探讨 被引量:4

摘要 目的:探讨二维超声三切面法在筛查胎儿先天性心脏畸形中的可行性及局限性。方法:选择我院2010年6月~2011年6月我院门诊接受产前检查的孕妇8800例,所有孕妇均采用二维超声三切面法筛查胎儿先天性心脏畸形,异常病例行超声心动图检查,结果与出生后或引产后尸检结果进行对照。结果:检出13例胎儿心脏畸形,占0.15%,均建议孕妇进行胎儿超声心动图进一步检查。经超声心动图复查后符合11例,误诊2例,误诊率15.38%;所有检查结果均经超声心动图及引产经尸检证实。分娩的新生儿中,临床听诊发现2例新生儿心脏杂音,经超声心动图检查确诊为室间隔缺损,漏诊率为0.02%。结论:二维超声三切面法操作简单,可快速直观地显示胎儿的心脏解剖结构,可作为产前筛查的工具,但有一定的局限性,检出率与胎儿的体位、孕周及医师技术有密切关系。 Objective:To study the two-dimensional ultrasound screening method three views fetal congenital cardiac anomaly of the feasibility and limitations. Methods:Choose our hospital in June 2010-June 2011 outpatient accept our antenatal examination of the pregnant women in 8800 cases, all pregnant women are using two-dimensional ultrasound screening method three views fetal congenital cardiac anomaly, abnormal cases do echocardiography, results and was born or induced labor autopsy results after comparison. Results:Detection 13 cases of fetal heart abnormalities, 0.15%, are recommended for pregnant women to fetal echocardiogram further examination. After checking by echocardiography with 11 cases, the misdiagnosis in 2 cases, the misdiagnosis rate was 13.33%;All inspection results both by echocardiography and induced labor confirmed by the autopsy. Delivery of the newborn, clinical auscultation found 2 newborns heart noise, the echocardiography diagnosis of ventricular septal defect, the missed diagnosis was 0.02%. Conclusion:Two-dimensional ultrasound method three views operation is simple, rapid and direct-viewing showing of the fetal heart anatomical structure, as a screening tool in prenatal, but there are some limitations, the detection rate and fetal gestational age at postures, there is a close relationship between doctors and technology.
