摘要
目的:探讨曼月乐和米非司酮治疗围绝经期功能失调性子宫出血临床效果。方法:收集2019年1月-2020年12月岱岳区妇幼保健计划生育中心治疗的800例围绝经期功能失调性子宫出血患者,随机分成试验组和对照组,两组患者均实施刮宫术,对照组于刮宫术后的第5天,口服米非司酮10 mg/d,治疗3个月,试验组在对照组的基础上联合曼月乐治疗,于刮宫术后的第5天,子宫内置入曼月乐,1粒/d,治疗3个月。结果:两组患者治疗前血清雌二醇(E_(2))、孕酮、促黄体生成素(LH)、促卵泡生成素(FSH)比较,差异无统计学意义(P>0.05),治疗后试验组明显降低,与对照组比较,差异有统计学意义(P<0.05)。治疗后试验组血红蛋白水平明显高于对照组,子宫内膜厚度和月经量明显低于对照组,差异均有统计学意义(P<0.05)。试验组总有效率明显高于对照组,差异有统计学意义(P<0.05)。结论:曼月乐和米非司酮治疗围绝经期功能失调性子宫出血可以明显改善患者的E_(2)、孕酮、LH、FSH水平,提高血红蛋白含量,降低子宫内膜厚度和月经量,疗效显著,值得推广。
Objective:To investigate the clinical effect of Mirena and Mifepristone in the treatment of perimenopausal dysfunctional uterine bleeding.Methods:A total of 800 patients with perimenopausal dysfunctional uterine bleeding treated in Maternal and Child Health and Family Planning Service Center of Daiyue District of Taian City from January 2019 to December 2020 were selected.They were randomly divided into experimental group and control group.The control group was given Mifepristone 10 mg/d orally on the 5th day after curettage for 3 months.The experimental group was given Mirena on the basis of control group,and the uterus was implanted with Mirena on the 5th day after curettage,1 capsule/d,for 3 months.Results:There was no significant difference in serum estradiol(E_(2)),progesterone,luteinizing hormone(LH)and follicle stimulating hormone(FSH)between the two groups before treatment(P>0.05).After treatment,the indicators in the experimental group were significantly lower than those in the control group,with statistically significant difference(P<0.05).After treatment,the hemoglobin level of the experimental group was significantly higher than that of the control group,and the endometrial thickness and menstrual volume of the experimental group were significantly lower than those of the control group,with statistically significant differences(P<0.05).The total effective rate of the experimental group was significantly higher than that of the control group,with statistically significant differences(P<0.05).Conclusion:In the treatment of perimenopausal dysfunctional uterine bleeding,Mirena and Mifepristone can significantly improve E_(2),progesterone,LH,FSH levels,increase hemoglobin content,reduce endometrial thickness and menstrual volume,with significant curative effect and worthy of promotion.
