摘要
目的探讨急性肾损伤(AKI)发生的影响因素并构建预测列线图。方法选取2018年6月至2020年6月在本院ICU住院的824例患者,根据患者是否合并AKI分为AKI组(307例)和非AKI组(517例)。收集患者的性别、年龄、民族以及原发疾病种类、疾病史、手术史等相关资料,对比构成分布并进行预测模型构建。结果logistic回归分析结果提示年龄、男性、合并慢性肾脏疾病、休克、脑卒中、泌尿道梗阻以及有手术史都是AKI发生的危险因素,女性患者中妊高症也是AKI发生的危险因素;进一步构建AKI相关危险因素的列线图,结果提示年龄≥70岁、男性、合并慢性肾脏疾病、休克、脑卒中、泌尿道梗阻以及既往有手术史都是AKI发生的高风险因素。结论年龄≥70岁、男性、合并慢性肾脏疾病、休克、脑卒中、泌尿道梗阻、脓毒血症以及既往有手术史是ICU患者发生AKI的高危因素,临床上应多关注此类患者。
Objective To investigate the influencing factors of acute kidney injury(AKI)and construction of predictive nomogram.Methods A total of 824 patients admitted to ICU in our hospital from June 2018 to June 2020 were selected and divided into AKI group(307 cases)and non-AKI group(517 cases)according to whether they were complicated with AKI.Collect the gender,age,nationality,type of primary disease,disease history,operation history and other relevant data of patients were collected,and compare the composition distribution was compared and the prediction model was constructed.Results Logistic regression analysis showed that age,male,chronic kidney disease,shock,stroke,urinary tract obstruction,sepsis and previous surgical history were all risk factors for AKI,while pregnancy induced hypertension in female patients was also a risk factor for AKI.Furthermore,the rogram of risk factors related to AKI was further constructed,suggesting that age≥70 years,male,chronic kidney disease,shock,stroke,urinary tract obstruction,sepsis and previous surgical history were all high-risk factors for AKI.Conclusions Age≥70 years old,male,complicated with chronic kidney disease,shock,stroke,urinary tract obstruction,sepsis and previous operation history are the high-risk factors of AKI in ICU patients.Clinical attention should be paid to these patients.
