基于医疗大数据的抗菌药物合理使用分析方法探讨与实践 被引量:2

摘要 随着数字赋能医院高质量发展的国家战略逐步推进,通过分析医疗大数据精准提升医院管理和诊疗能力已经成为医院核心能力。针对各家医院普遍存在的抗菌药物合理使用水平难以量化分析的痛点,本文通过融合多个数据源构建了包含71051名住院患者抗菌药物使用记录的医疗数据库,使用非参数检验、CV值分析、Z值分析三种数据分析方法,从抗菌药物使用的影响因素、疾病组抗菌物药使用规范性、医疗组抗菌药物使用合理性三个维度,开展深入全面的量化分析。通过研究发现,药物过敏史、年龄、诊疗难度、诊疗风险、诊疗方式、手术等级对患者抗菌药物使用差异存在统计学意义(p<0.05)。通过研究,建立疾病组抗菌药物规范使用评估模型和医疗组抗菌药物合理使用水平评估模型。通过三种分析方法的组合应用,为医院抗菌药物合理使用的持续改进,提供充分必要的量化数据支撑。 With the advancement of the national strategy of high-quality development of digital enabled hospitals,it has become the core competence of hospitals to improve hospital management and diagnosis and treatment ability accurately through analyzing medical big data.Aiming at the difficulty of quantitative analysis of rational antibiotics use,this paper constructed a large medical database of antibiotics use records of 71051 inpatients by integrating multiple data sources.Three data analysis methods including nonparametric test,CV value analysis and Z value analysis were used to carry out in-depth and comprehensive quantitative analysis from three dimensions:the influencing factors of antimicrobial use,the standardization of antimicrobial use in the disease group and the rationality of antimicrobial use in the medical group.The study found that drug allergy history,age,difficulty of diagnosis and treatment,risk of diagnosis and treatment,type of treatment,and surgery level were influencing factors of antibiotics use(p<0.05).The evaluation model of rational antibiotics use in disease group and the evaluation model of rational antibiotics use in medical group were established.The combined application of the three analysis methods provided sufficient and necessary quantitative data support for the continuous improvement of the rational antibiotics use in hospitals.
