糖尿病肾病终末期患者血液透析治疗对营养状态及微炎症状态的影响 被引量:4

Effects of Hemodialysis Treatment on Nutritional Status and Micro-inflammatory Status in Patients with End-stage Diabetic Nephropathy

摘要 目的研究糖尿病肾病终末期患者血液透析治疗对营养状态及微炎症状态的影响。方法选择2019年10月—2020年10月期间在该院就诊的76例糖尿病肾病终末期患者展开研究,按照随机数表法进行分组,对照组38例予以低通量血液透析,观察组38例实施高通量血液透析。比较两组的治疗效果。结果透析后观察组的空腹血糖、餐后2 h血糖、C反应蛋白(CRP)、白细胞介素6(IL-6)、肿瘤坏死因子-α(TNF-α)水平低于对照组,透析后的血清总蛋白(TP)、血清白蛋白(ALB)、转运蛋白(TRF)高于对照组,差异有统计学意义(P<0.05)。观察组的尿素氮(BUN)、血肌酐(Scr)、血清β2微球蛋白(β2-MG)低于对照组,差异有统计学意义(P<0.05)。结论同低通量血液透析相比,高通量血液透析可有效改善糖尿病肾病终末期患者的血糖水平、营养状态以及肾功能,减轻微炎症反应。 Objective To study the effect of hemodialysis treatment on nutritional status and micro-inflammatory status in patients with end-stage diabetic nephropathy.Methods This study included 76 patients with end-stage diabetic nephropathy who were treated in the hospital from October 2019 to October 2020.According to the random number table method,38 patients in the control group received low-flux hemodialysis,and 38 patients in the observation group received high-flux hemodialysis.Compared the treatment effects of the two groups.Results The fasting blood glucose,2 h postprandial blood glucose,serum total protein(TP),serum albumin(ALB),transferrin(TRF)levels of the observation group after dialysis were lower than those of the control group,and the C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)after dialysis were higher than those of the control group,the difference was statistically significant(P<0.05).The urea nitrogen(BUN),serum creatinine(Scr),serumβ2 microglobulin(β2-MG)of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with low-flux hemodialysis,high-flux hemodialysis can effectively improve the blood glucose level,nutritional status and renal function of patients with end-stage diabetic nephropathy,and reduce the micro-inflammatory response.
