环磷酰胺联合氢氯噻嗪治疗2型糖尿病肾病的临床研究 被引量:1

Clinical Study of Cyclophosphamide Combined with Hydrochlorothiazide in the Treatment of Type 2 Diabetic Nephropathy

摘要 目的研究环磷酰胺联合氢氯噻嗪治疗2型糖尿病肾病的应用效果。方法选取2020年5月-2021年5月于都县人民医院收治的60例2型糖尿病肾病患者作为研究对象,按照随机数字表法分为对照组与观察组,各30例。对照组给予氢氯噻嗪治疗,观察组给予环磷酰胺联合氢氯噻嗪治疗,比较两组治疗效果、肾功能指标[尿白蛋白排泄率(UAER)、血清β2微球蛋白(β2-MG)、血尿素氮(BUN)]、血清同型半胱氨酸(Hcy)、胱抑素C(Cys C)、N末端B型利钠肽原(NT-proBNP)及不良反应发生情况。结果观察组治疗总有效率为93.33%,高于对照组的80.00%(P<0.05)。两组治疗后UAER、β2-MG、BUN水平均低于治疗前,且观察组低于对照组(P<0.05)。两组治疗后Hcy、Cys C、NT-proBNP水平均低于治疗前,且观察组低于对照组(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论环磷酰胺联合氢氯噻嗪治疗2型糖尿病肾病疗效确切,可改善患者肾功能,调节炎症反应及免疫反应,且联合用药不增加不良反应发生几率。 Objective To study the application effect of cyclophosphamide combined with hydrochlorothiazide in the treatment of type 2 diabetic nephropathy.Methods A total of 60 patients with type 2 diabetic nephropathy admitted to Yudu County People's Hospital from May 2020 to May 2021 were selected as the study subjects.They were divided into control group and observation group according to the random number table method,with 30 cases in each group.The control group was treated with hydrochlorothiazide,while the observation group was treated with cyclophosphamide combined with hydrochlorothiazide.The therapeutic effect,renal function indexes[urinary albumin excretion rate(UAER),serumβ2 microglobulin(β2-MG),blood urea nitrogen(BUN)],serum homocysteine(Hcy),cystatin C(Cys C),N-terminal B-type natriuretic peptide(NT-proBNP)and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 93.33%,which was higher than 80.00%of the control group(P<0.05).After treatment,the levels of UAER,β2-MG and BUN in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of Hcy,Cys C and NT-proBNP in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Cyclophosphamide combined with hydrochlorothiazide is effective in the treatment of type 2 diabetic nephropathy,which can improve renal function,regulate inflammatory response and immune response,and does not increase the incidence of adverse reactions.
