摘要
目的探讨在终末期糖尿病肾病患者血透治疗中以细节管理护理的效果。方法选取2020年9月—2022年3月在南京市溧水区中医院接受血透治疗的50例终末期糖尿病肾病患者为研究对象,采取随机信封法分为两组,将接受常规护理管理的25例患者设为对照组,另一组25例采用细节管理护理的设为观察组,对比两组患者情绪变化情况,并发症发生率和生活质量指标情况。结果护理前,两组情绪评分对比,差异无统计学意义(P>0.05);护理后,观察组焦虑、抑郁评分分别(40.28±2.55)分、(42.65±3.04)分,均低于对照组,差异有统计学意义(t=7.406、6.118,P<0.05)。观察组并发症发生率0.00%低于对照组的24.00%,差异有统计学意义(χ^(2)=4.735,P<0.05)。观察组认知、角色、躯体、社会、情绪等领域评分分别(81.82±2.61)分、(82.73±2.31)分、(81.42±1.63)分、(80.44±1.73)分、(82.37±2.15)分,均高于对照组,差异有统计学意义(t=3.117、6.045、7.601、6.140、4.545,P<0.05)。结论终末期糖尿病肾病患者血透治疗中以细节护理管理进行干预,可对患者不良心理进行改善,减少并发症发生的同时,还能起到改善预后的作用。
Objective To explore the effect of detail management nursing in hemodialysis treatment of patients with end-stage diabetic nephropathy.Methods A total of 50 patients with end-stage diabetes and nephropathy who received hemodialysis treatment in Lishui District Hospital of Traditional Chinese Medicine from September 2020 to March 2022 were studied as the research object and divided into two groups by random envelope method.25 patients receiving routine nursing management were set as control group,and another group of 25 patients receiving detailed management nursing was set as observation group.Mood change,complication rate and quality of life index were compared between the two groups.Results Before nursing,there was no statistically significant difference between the two groups in emotional scores(P>0.05).After nursing,the scores of anxiety and depression in the observation group were(40.28±2.55)points and(42.65±3.04)points respectively,which were lower than those in the control group,and the difference was statistically significant(t=7.406,6.118,P<0.05).The complication rate in the observation group was 0.00%lower than that in the control group(24.00%),and the difference was statistically significant(χ^(2)=4.735,P<0.05).The scores of cognitive,role,body,social and emotional fields in the observation group were(81.82±2.61)points,(82.73±2.31)points,(81.42±1.63)points,(80.44±1.73)points and(82.37±2.15)points respectively,which were higher than those in the control group,and the difference was statistically significant(t=3.117,6.045,7.601,6.140,4.545,P<0.05).Conclusion Detailed nursing management intervention in hemodialysis treatment of end-stage diabetic nephropathy patients can improve the adverse psychology of patients,reduce the occurrence of complications,but also play a role in improving the prognosis.
