摘要
目的探讨格列本脲联合胰岛素治疗妊娠糖尿病的临床效果及对不良妊娠结局发生情况的影响。方法选取2020年4月至2021年4月本院收治的86例妊娠糖尿病患者,采用随机数字表法将其分为对照组和观察组,各43例。对照组采用胰岛素治疗,观察组在对照组基础上联合格列本脲治疗。比较两组的Hcy、Irisin、GAL、凝血功能指标、FABP4水平、PAPP-A中位数倍数及不良妊娠结局发生情况。结果治疗后,两组患者的Hcy、GAL水平均降低,Irisin水平均升高,且观察组优于对照组(P<0.05)。治疗后,观察组的PT、APTT长于对照组,FBG、FABP4水平低于对照组,PAPP-A中位数倍数大于对照组(P<0.05)。观察组产妇及新生儿的不良妊娠结局总发生率分别为6.98%、4.65%,低于对照组的23.26%、18.60%(P<0.05)。结论格列本脲联合胰岛素治疗妊娠糖尿病的效果显著,可降低不良妊娠结局发生率。
Objective To investigate the clinical effect of glibenclamide combined with insulin in the treatment of gestational diabetes mellitus and its influence on adverse pregnancy outcomes.Methods A total of 86 patients with gestational diabetes mellitus admitted in our hospital from April 2020 to April 2021 were selected and divided into control group and observation group by random number table method,with 43 cases in each group.The control group was treated with insulin,and the observation group was treated with glibenclamide on the basis of the control group.The levels of Hcy,irisin,GAL,indexes of coagulation function,FABP4,median multiples of PAPP-A and adverse pregnancy outcomes were compared between the two groups.Results After treatment,the levels of Hcy and GAL decreased and irisin level increased in both groups,and those in the observation group were better than the control group(P<0.05).After treatment,the PT,APTT in the observation group were longer than those in the control group,the levels of FBG and FABP4 were lower than those in the control group,and median multiples of PAPP-A was higher than that in the control group(P<0.05).The total incidence of maternal and neonatal adverse pregnancy outcomes in the observation group were 6.98% and 4.65% respectively,which were lower than 23.26% and 18.60% in the control group(P<0.05).Conclusion Glibenclamide combined with insulin has significant effect in treating gestational diabetes mellitus,which can reduce the incidence of adverse pregnancy outcomes.
