格列本脲治疗妊娠期糖尿病及对胰岛β细胞和围生儿结局的影响 被引量:18

Effects of glibenclamide on gestational diabetes mellitus and the outcome of islet beta cells and perinatal infants

摘要 目的探究格列本脲治疗妊娠期糖尿病(GDM)的疗效及对产妇β细胞功能和围生儿结局的影响。方法选择198例GDM产妇随机分为观察组与对照组,各99例。对照组使用胰岛素治疗,观察组在对照组基础上加用格列本脲治疗。检测治疗前后2组血清糖化血红蛋白(HbAlc)、血清胱抑素C(Cys C)水平;进行口服葡萄糖耐量试验(OGTT)和胰岛素释放试验,以稳态模型评估β细胞功能指数(HOMA-β)和胰岛素抵抗指数(HOMA-IR);统计2组新生儿围生儿结局。结果治疗后,观察组HbAlc及Cys C水平低于对照组(P<0.05),HOMA-β及HOMA-IR水平低于对照组(P<0.05);观察组新生儿低血糖症、高胆红素血症、5 min Apgar评分<7分及巨大儿发生率低于对照组(P<0.05)。结论格列本脲联合胰岛素治疗能够显著降低GDM产妇的血糖水平,改善肾功能和胰岛β细胞分泌功能,改善围生儿结局,值得临床推广使用。 Objective To investigate the effect of glibenclamide on beta cell function and perinatal outcomes of pregnant women with gestational diabetes mellitus.Methods A total of 198 parturients with gestational diabetes mellitus who were treated in our hospital were divided into experimental group and control group according to the random number table method,99 parturients in each group.The control group were given insulin as routine treatment.The experimental group were given Glien on the basis of the control group.The serum levels of HbAlc and Cys C were compared between the two groups before and after treatment.The 75 g oral glucose tolerance test and insulin release test were performed in the two groups.The homeostasis model was used to evaluate the beta cell function index(HOMA-beta)and homeostasis model was used to evaluate insulin resistance.The index(HOMA-IR)was compared before and after treatment.The perinatal outcomes of the two groups were statistically analyzed.Results The HbAlc and Cys C level in the experimental group were lower than those in the control group(P<0.05);the incidence of hypoglycemia,hyperbilirubinemia,Apgar score<7 at 5 min and macrosomia in the experimental group were lower than those in the control group(P<0.05).Conclusion Glibenclamide could significantly improve the clinical symptoms of gestational diabetes mellitus.At the same time,the secretion function of pancreatic islet beta cells was improved,insulin resistance is decreased,and the perinatal outcome was better.
