SLE患者血清miR-202-3p、PS-PLA1水平与疾病活动度和细胞免疫功能的关系 被引量:5

摘要 目的探讨系统性红斑狼疮(SLE)患者血清微小RNA-202-3p(miR-202-3p)、磷脂酰丝氨酸特异性磷脂酶A1(PS-PLA1)水平与疾病活动度和细胞免疫功能的关系。方法选择SLE患者158例(SLE组),根据SLE疾病活动度指数(SLEDAI)评分分为轻度52例、中度71例、重度35例,同期选择体检健康的志愿者76例(对照组),采集所有研究对象空腹肘静脉血,离心留取血清,采用RT-qPCR法检测血清miR-202-3p,采用ELISA法检测血清PS-PLA1,采用流式细胞仪检测细胞免疫功能(CD3^(+)、CD4^(+)、CD8^(+)T细胞以及CD4^(+)/CD8^(+))。采用Pearson或Spearman相关分析法分析SLE患者血清miR-202-3p、PS-PLA1水平与SLEDAI评分和细胞免疫功能的关系。采用受试者工作特征(ROC)曲线分析血清miR-202-3p、PS-PLA1水平对重度SLE的鉴别诊断价值。结果观察组血清miR-202-3p水平低于对照组,血清PS-PLA1水平高于对照组(P均<0.05)。随着疾病活动度增加,SLE患者血清miR-202-3p水平逐渐降低,血清PS-PLA1水平逐渐升高(P均<0.05)。观察组CD3^(+)、CD4^(+)T细胞及CD4^(+)/CD8^(+)均低于对照组,而CD8^(+)T细胞高于对照组(P均<0.05)。相关性分析显示,SLE患者血清miR-202-3p水平与SLEDAI评分、CD8^(+)T细胞均呈负相关关系,与CD3^(+)、CD4^(+)T细胞及CD4^(+)/CD8^(+)均呈正相关关系(P均<0.05);SLE患者血清PS-PLA1水平与SLEDAI评分、CD8^(+)T细胞均呈正相关关系,与CD3^(+)、CD4^(+)T细胞及CD4^(+)/CD8^(+)均呈负相关关系(P均<0.05)。血清miR-202-3p、PS-PLA1水平单独时鉴别重度SLE的曲线下面积(AUC)分别为0.697、0.717,二者联合时鉴别重度SLE的AUC为0.903,血清miR-202-3p、PS-PLA1水平联合时鉴别重度SLE的AUC高于二者单独时(Z分别为4.417、3.311,P均<0.05)。结论SLE患者血清miR-202-3p水平降低、血清PS-PLA1水平升高,并且二者与疾病活动度和细胞免疫功能紊乱密切相关。miR-202-3p、PS-PLA1可作为鉴别SLE病情程度的血清生物标志物。 Objective To investigate the relationships between serum microRNA-202-3p(miR-202-3p),phosphatidylserine-specific phospholipase A1(PS-PLA1)levels and disease activity and cellular immune function in patients with systemic lupus erythematosus(SLE).Methods Totally 158 SLE patients(SLE group)were selected.According to the SLE disease activity index(SLEDAI),52 cases were mild,71 were moderate,and 35 were severe,and meanwhile,76 healthy volunteers(control group)were selected at the same time.Fasting elbow venous blood was collected and serum was collected by centrifugation.Serum miR-202-3p was detected by RT-qPCR,serum PS-PLA1 was detected by ELISA,and cellular immune function(CD3^(+),CD4^(+),CD8^(+)T cells and CD4^(+)/CD8^(+))was detected by flow cytometry.Pearson or Spearman correlation analysis was used to analyze the relationship between serum miR-202-3p and PS-PLA1 levels and SLEDAI score and cellular immune function in SLE patients.Receiver operating characteristic(ROC)curve was used to analyze the differential diagnosis value of serum miR-202-3p and PS-PLA1 levels in severe SLE.Results The serum miR-202-3p level in the observation group was lower than that in the control group,and the serum PS-PLA1 level was higher than that in the control group(both P<0.05).With the increase of disease activity,serum miR-202-3p level gradually decreased and serum PS-PLA1 level gradually increased in SLE patients(all P<0.05).CD3^(+),CD4^(+)T cells and CD4^(+)/CD8^(+)were lower in the observation group than in the control group,while CD8^(+)T cells were higher than that of the control group(all P<0.05).Correlation analysis showed that serum miR-202-3p level was negatively correlated with SLEDAI score and CD8^(+)T cells,and was positively correlated with CD3^(+),CD4^(+)T cells and CD4^(+)/CD8^(+)(all P<0.05).The serum PS-PLA1 level of SLE patients was positively correlated with SLEDAI score and CD8^(+)T cells,and was negatively correlated with CD3^(+),CD4^(+)T cells and CD4^(+)/CD8^(+)(all P<0.05).The area under the curve(AUC)of s
