多重微珠流式免疫荧光法检测系统性红斑狼疮患者自身抗体的价值

The clinical application of multi-bead flow immunofluorescence technology for detecting autoantibodies in patients with systemic lupus erythematosus

摘要 目的探讨多重微珠流式免疫荧光法检测系统性红斑狼疮(SLE)患者自身抗体的结果和应用价值。方法选取2020年10月-2021年3月广东省中医院大学城医院门诊或住院确诊为SLE的患者46例(试验组),健康体检者30名(对照组),利用多重微珠流式免疫荧光法和免疫印迹法检测自身抗体并分析结果,包括抗组蛋白抗体、抗着丝点抗体、抗U1核糖核蛋白(U1-RNP)抗体、抗Sm抗体、抗SSA抗体、抗SSB抗体、抗拓扑异构酶(Scl-70)抗体、抗组氨酰转运核糖核酸合成酶(Jo-1)抗体、抗双链DNA(dsDNA)抗体。结果试验组采用多重微珠流式免疫荧光法和免疫印迹法检测抗核抗体谱(ANAs)阳性率分别为73.9%和80.4%,差异无统计学意义(χ^(2)=0.80,P>0.05);但抗U1-RNP抗体阳性率分别为28.2%和47.8%,差异有统计学意义(χ^(2)=4.97,P<0.05)。利用多重微珠流式免疫荧光法检测SLE患者和健康体检者ANAs,其中抗组蛋白抗体、抗U1-RNP抗体、抗Sm抗体、抗SSA抗体、抗SSB抗体、抗Scl-70抗体、抗dsDNA抗体水平比较差异均有统计学意义(P均<0.05),抗着丝点抗体和抗Jo-1抗体水平比较差异无统计学意义(P>0.05)。结论多重微珠流式免疫荧光法可以一次性同时进行多种自身抗体的联合检测,且用该方法与现有常规方法检测SLE患者自身抗体具有一致性,能为临床提供良好实验室数据。 Objective To analyze the results and clinical application of multi-bead flow immunofluorescence method for detecting autoantibodies in patients with systemic lupus erythematosus(SLE).Methods A total of 46 patients with SLE(experimental group)and 30 healthy individuals(control group)were selected from the Guangdong Province Hospital of Traditional Chinese Medicine outpatient or inpatient from October 2020 to March 2021.Multiple bead flow immunofluorescence and immunoblot were used to detect autoantibodies,including anti-histones antibody,anti-centromere antibody,anti-U1-RNP antibody,anti-Sm antibody,anti-SSA antibody,anti-SSB antibody,anti-Scl-70 antibody,anti-Jo-1antibody,and anti-double-strand DNA(anti-dsDNA)antibody.Results In the experimental group,the positive rates of antinuclear antibody spectrum(ANAs)detected by multi-bead flow immunofluorescence and immunoblot were 73.9%and80.4%,respectively,and the difference was not statistically significant(χ^(2)=4.80,P>0.05).However,the positive rates of anti-U1-RNP antibodies detected by multi-bead flow immunofluorescence and immunoblot were 28.2%and 47.8%,respectively,and the difference was statistically significant(χ^(2)=4.97,P<0.05).The multi-bead flow immunofluorescence method was used to detect the two groups of ANAs,and the differences in the levels of anti-histone antibody,anti-U1-RNP antibody,anti-Sm antibody,anti-SSA antibody,anti-SSB antibody,anti-Scl-70 antibody,and anti-dsDNA antibody were statistically significant(P all<0.05).The differences in the level of anti-centromere antibody and anti-Jo-1 antibody were not statistically significant(P>0.05).Conclusion The multi-bead flow immunofluorescence method could perform the combined detection of multiple autoantibodies at the same time,and the method was consistent with the existing conventional methods for detecting autoantibodies in SLE patients,which could provide good clinical indicators.
