ANA、抗-ENA和抗-dsDNA抗体对SLE诊断的意义 被引量:5

Significance of detection of ANA,anti-ENA,anti-dsDNA natibody in diagnosis of SLE

摘要 目的探讨联合检测抗核抗体(ANA)、抗可提取性抗原抗体(抗ENA)及抗双链DNA抗体的临床价值。方法采用间接免疫荧光检测ANA,采用胶体金斑点渗透法检测抗ds-DNA抗体,谱采用免疫印迹法检测抗ENA、抗Sm、抗nRNP/Sm、抗SS-A、抗SS-B、抗Scl-70、抗Jo-1。结果 SLE组ANA阳性率为70.8%,抗ds-DNA抗体阳性率为47.5%,抗Sm抗体阳性率为37.5%,抗nRNP/Sm抗体阳性率为29.16%,抗SS-A抗体阳性率为26.66%,抗SS-B抗体阳性率为21.66%,抗Scl-70抗体阳性率为1.66%,抗Jo-1抗体阳性率为0%,对照组中只有ANA的阳性率为6.67%,其他抗体均为阴性用间接免疫荧光法检测ANA核颗粒型、核均质型、浆颗粒型、核仁型、着丝点型分别为62.4%、23.5%,10.6%,2.4%,1.2%。结论 ANA、抗ds-DNA抗体和抗Sm抗体可作为检测的标志性抗体,联合检测可提高检测率,对病情的诊断治疗、追踪和预后判定均有重要价值。 Aim To investigate the significane of detection of anti-nuclear antibody(ANA),anti-extractable-antigen antibody(anti-ENA)and the anti-double strand DNA antibody in diagnosis of systemic erythematosus lupus(SLE). Methods ANA was detected with indirect immunity fluorescence test,the anti-ds-DNA was detected with immunogold assay,anti-ENA,anti-Sm,anti-nRNP/Sm,anti-SS-A,anti-SS-B,anti-Scl-70,anti-Jo-1 were detected with.Western blot. Results The positive rates of ANA,anti-ds-DNA,anti-Sm,anti-nRNP/Sm,anti-SS-A,anti-SS-B,anti-Scl-70 and anti-Jo -1 were 70.8%,47.5%,37.5%,29.16%26.66%,21.66%,1.66%and 0%in SLE patiente.While only ANA was positive in the control group with a positive rate of 6.67%.Conclusion ANA,anti-ds-DNA and anti-Sm can be taken as the indicator antibodies in diagnosis of SLE and joint detection can enhance diagnositc rate in addition to valuable for guidance of treatment,tracing and prognosis of the disease.
