摘要 目的:探讨早期持续被动运动训练配合关节松动术对全膝关节置换术(TKA)后恢复的影响。方法:选取2015年1月-2020年1月广州市第一人民医院膝骨关节炎行TKA患者60例。采用随机数字表法将其分成常规组与研究组,各30例。研究组于术后第3天开始行持续被动运动训练联合关节松动术,常规组术后3 d开始进行单纯关节松动术训练。比较两组术后3、14、30 d膝关节肿胀和疼痛程度,膝关节功能。比较两组术前,术后15、30 d膝关节活动度。结果:两组术后3 d关节肿胀程度,视觉模拟评分法(VAS)、美国特种外科医院(HSS)评分比较差异均无统计学意义(P>0.05),两组术后14、30 d的关节肿胀程度,VAS、HSS评分均优于术后3 d(P<0.05),研究组术后14、30 d关节肿胀程度、VAS评分均低于常规组,HSS评分高于常规组(P<0.05)。术后15、30 d,研究组膝关节屈曲度均大于常规组,伸直度均小于常规组(P<0.05)。结论:早期持续被动运动训练配合关节松动术可减轻TKA患者术后关节肿胀程度,缓解疼痛症状,加速早期膝关节功能恢复,值得临床推广。 Objective:To explore the effect of early continuous passive exercise training combined with joint mobilization on recovery after total knee arthroplasty(TKA).Method:A total of 60 patients with knee osteoarthritis who underwent TKA in Guangzhou First People’s Hospital from January 2015 to January 2020 were selected.Random number table method was used to divide them into conventional group and study group,with 30 cases in each group.The study group began to receive continuous passive exercise training combined with joint mobilization on the third day after surgery,while the conventional group began to receive simple joint mobilization training 3 days after surgery.The degree of knee joint swelling and pain and knee function were compared between the two groups at 3,14 and 30 d after surgery.The range of motion of knee joint of the two groups were compared before and 15,30 d after surgery.Result:There were no significant differences in the degree of joint swelling,visual analog scale(VAS)and hospital for special surgery(HSS)scores between the two groups at 3 d after surgery(P>0.05).The degree of joint swelling,VAS and HSS scores of the two groups at 14 and 30 d after surgery were better than those at 3 d after surgery(P<0.05).The joint swelling degree and VAS score of the study group were lower than those of the conventional group at 14 and 30 d after surgery,and HSS score was higher than that of the conventional group(P<0.05).At 15 and 30 d after surgery,the degree of knee joint flexion of the study group was greater than that of the conventional group,the degree of knee joint extension was less than that of conventional group(P<0.05).Conclusion:Early continuous passive exercise training combined with joint mobilization can reduce the degree of postoperative joint swelling in TKA patients,relieve pain symptoms,and accelerate the early recovery of knee joint function.It is worthy of clinical promotion.
