摘要
目的观察针刺联合CPM机对全膝关节置换术(TKA)后关节疼痛、关节肿胀及关节活动度的影响。方法将2019年6月—2020年4月陕西中医药大学附属医院收治的60例行单膝关节置换术后患者随机分为对照组、观察1组、观察2组,每组各20例。对照组采取CPM机功能锻炼,观察1组采取CPM机配合关节松动术治疗,观察2组采取CPM机配合针刺治疗,3组均干预2周。观察比较3组患者术后1 d、3 d、7 d、14 d膝关节疼痛评分(VAS)、关节肿胀程度和关节活动度(ROM)。结果术后3 d、7 d、14 d观察2组疼痛评分、肿胀程度均明显低于观察1组、对照组(P均<0.05);观察2组关节活动度优于观察1组、对照组(P均<0.05)。结论全膝关节置换术后给予针刺联合CPM机治疗近期镇痛效果更显著,关节活动度范围更大。
Objective It is to observe the effect of acupuncture combined with CPM machine on joint pain,joint swelling and joint range of motion after total knee arthroplasty(TKA).Methods Sixty patients undergoing single knee arthroplasty who were admitted to the Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine from June 2019 to April 2020 were randomly divided into 3 groups:control group,observation group I,and observation groupⅡ,with 20 cases in each group.The control group was treated with CPM machine for functional exercise,the observation group I was treated with CPM machine combined with manipulative joint loosening,the observation groupⅡwas treated with CPM machine combined with acupuncture,and the three groups all intervened for 2 weeks.The knee joint pain score(VAS),the degree of joint swelling and the range of motion(ROM)of the patients of the 3 groups were observed and compared at 1 d,3 d,7 d,14 d after operation.Results After 3 d,7 d,and 14 d,the pain score and swelling degree of the observation group II were significantly lower than those of observation group I and control group(all P<0.05);the joint mobility of observation group II was better than observation group I and control group(all P<0.05).Conclusion Acupuncture combined with CPM machine has a more significant short-term analgesic effect and a greater range of joint range of motion after total knee arthroplasty.
