摘要
目的探究反复种植失败(RIF)后冻融胚胎移植(FET)时行宫腔灌注重组人绒毛膜促性腺激素(rHCG)对妊娠结局的影响。方法选取2019年5月至2020年5月邯郸市中心医院诊治的101例因RIF行FET的患者作为研究对象,根据FET前是否行宫腔灌注rHCG分为研究组(n=51)和对照组(n=50)。研究组FET前15min行宫腔灌注rHCG,对照组行宫腔灌注培养液。比较两组子宫内膜血流参数[搏动指数(PI)、阻力指数(RI)],以及宫腔液中血管内皮生长因子(VEGF)、嗜酸性粒细胞趋化因子(Eotaxin)、γ干扰素(IFN-γ)水平,并比较两组妊娠结局。结果研究组移植时PI、RI低于对照组(P<0.05);研究组移植时宫腔液VEGF、Eotaxin水平高于对照组,IFN-γ水平低于对照组(P<0.05);研究组胚胎着床率(52.33%)、β-HCG阳性率(74.51%)、临床妊娠率(68.63%)均高于对照组(37.21%、52.00%、48.00%)(P<0.05)。结论RIF患者FET时行宫腔灌注rHCG能有效改善妊娠结局,具有较高的临床应用价值。
Objective To explore the effect of intrauterine perfusion of recombinant human chorionic gonadotropin(rHCG)on pregnancy outcome during freeze-thawed embryo transfer(FET)after repeated implant failure(RIF).Methods 101 RIF patients in Handan Central Hospital were selected,all underwent FET.According to whether rHCG was perfused into the uterine cavity before FET,they were divided into study group(n=51)and control group(n=50).The study group received rHCG perfusion in uterine cavity 15 minutes before FET,the control group was perfused with culture fluid in the uterine cavity.Endometrial blood flow parameters[pulsation index(PI),resistance index(RI)],vascular endothelial growth factor(VEGF),eosinophil chemotactic factor(Eotaxin),interferon-γ(IFN-γ)level,and the pregnancy outcomes were compare between the two groups.Results PI and RI during transplantation in the study group were lower than those in the control group(P<0.05).The levels of VEGF and Eotaxin in uterine fluid during transplantation in the study group were higher than those in the control group,while the levels of IFN-γwere lower than those in the control group(P<0.05).The embryo implantation rate(52.33%),β-HCG positive rate(74.51%)and clinical pregnancy rate(68.63%)in the study group were higher than those in the control group(37.21%,52.00%,48.00%)(P<0.05).Conclusions The intrauterine infusion of rHCG during FET in RIF patients can effectively improve the pregnancy outcome and has high clinical application value.
