摘要
目的探讨不同促排方案的冻融胚胎移植妊娠结局。方法回顾性分析2017年12月至2020年2月杭州市妇产科医院生殖医学中心FET周期的临床结局指标,按照取卵周期的促排方案分为三组:A组采用GnRH-a长方案,B组采用黄体酮+HMG方案,C组采用CC+HMG方案,比较各组的临床妊娠率、种植率和多胎率。结果三组患者的平均年龄比较,差异有统计学意义(P<0.05),A组患者平均年龄为(31.4±11.52)岁,显著低于B组和C组,差异有统计学意义(P<0.05)。A组的妊娠率(54.88%)、着床率(38.56%)均显著高于B组和C组,A组多胎率也显著高于C组,差异有统计学意义(P<0.05)。年龄分层后,三组患者的妊娠率比较,差异无统计学意义(P>0.05),但在部分年龄层,A组的胚胎着床率仍高于B组和C组,差异有统计学意义(P<0.05)。结论在冻融胚胎移植周期中,微刺激方案和长方案的妊娠结局无显著差异。对于微刺激方案和部分长方案患者,根据患者情况选择全胚冷冻后冻融胚胎移植也可达到较好的妊娠结局。
Objective To explore the pregnancy outcome of freeze-thaw embryo transfer with different ovulation induction protocols.Methods Clinical outcome indicators for FET cycles in Reproductive Medicine Center in Obstetrics and Gynecology Hospital of Hangzhou City from December 2017 to February 2020 were retrospectively analyzed,and divided into 3 groups according to the ovulation induction protocols of the oocyte retrieval cycle.These groups were group A GnRH-a long protocol,group B progesterone+HMG and group C CC+HMG.The clinical pregnancy rate,implantation rate and multiple pregnancy rate in each group were compared.Results There was significantly difference of mean age of patients among the three groups(P<0.05),and the mean age of patients in A group was(31.4±11.52)years old,which was significantly lower than that in B group and C group(P<0.05).The pregnancy rate(54.88%)and the implantation rate(38.56%)in A group were significantly higher than those in B group and C group,and the multiple pregnancy rate in A group was also significantly higher than that in C group.After age stratification,there was no significant difference in terms of pregnancy rate among the three groups,but the embryo implantation rate was still higher in A group than that in B group and C group in some age groups.Conclusion There was no significant difference in terms of pregnancy outcome between the microstimulation protocol and the long protocol during the freeze-thaw embryo transfer cycle.For patients in the microstimulation protocol and some in the long protocol,whole embryo freezing followed by freeze-thaw embryo transfer may also be an option depending on the patient’s condition to achieve a better pregnancy outcome.
