循证医学指导下PDCA循环护理在EICU中心静脉置管患者中的应用效果 被引量:2

摘要 目的:观察循证医学指导下PDCA循环护理在急诊重症监护病房(EICU)中心静脉置管患者中的应用效果。方法:将2018年7月至2019年7月该院收治的98例中心静脉置管患者纳入对照组(实施常规护理),2019年8月至2020年6月收治的98例中心静脉置管患者纳入观察组(实施循证医学指导下PDCA循环护理)。比较两组中心静脉置管感染发生率、导管留置时间、EICU入住时间和实施循证医学指导下PDCA循环护理前后护理人员预防导管感染知信行评分。结果:两组中心静脉置管感染发生率比较,差异无统计学意义(P>0.05);观察组导管留置时间长于对照组,EICU入住时间短于对照组,差异均有统计学意义(P<0.05);实施后,护理人员预防导管相关感染态度、行为、感染风险识别与预防能力、导管留置适应证掌握情况评分均高于实施前,差异有统计学意义(P<0.05)。结论:循证医学指导下PDCA循环护理能延长患者导管留置时间、缩短EICU入住时间,并提高护理人员预防导管感染知信行评分。 Objective:To observe application effects of evidence-based medicine-guided PDCA cycle nursing in emergency intensive care unit(EICU)patients with central venous catheterization.Methods:98 patients with central venous catheters who were treated in this hospital from July 2018 to July 2019 were included in control group(conventional nursing),while 98 patients with central venous catheters who were admitted from August 2019 to June 2020 were included in observation group(evidence-based medicine-guided PDCA cycle nursing).The incidence of central venous catheter infection,the catheter indwelling time,the EICU stay time,and the nursing staff’s knowledge,belief,and behavior scores for catheter infection prevention before and after the implementation of the evidence-based medicine-guided PDCA cycle nursing the were compared between the two groups.Results:There was no significant difference in the incidence of central venous catheter infection between the two groups(P>0.05).The indwelling time of central venous catheter in the observation group was longer than that in the control group;the EICU stay time was shorter than that in the control group;and the differences were statistically significant(P<0.05).After the implementation,the scores of the nursing staff’s attitude,behavior,infection risk identification and prevention ability,and the mastery of indications for catheter indwelling were higher than those before implementation,and the differences were statistically significant(P<0.05).Conclusions:The evidence-based medicine-guided PDCA cycle nursing for the EICU patients with central venous catheterization can prolong the central venous catheter indwelling time,shorten the EICU stay time,and improve the scores of nursing staff’s knowledge,belief,and behavior scores for catheter infection prevention.
