摘要
目的调查慢性鼻窦炎鼻内镜术后鼻腔填塞患者的睡眠质量状况及影响因素。方法于2020年7月至2021年3月,便利选取衡阳市南华大学附属第一医院耳鼻喉科360例慢性鼻窦炎鼻内镜术后鼻腔填塞患者为研究对象,调查工具为改良版Kolcaba舒适状况量表、睡眠状况自评量表、疼痛数字评分法,统计分析采用描述性统计、多元线性回归等方法。结果患者术后疼痛得分为(5.34±1.54)分,轻、中、重度疼痛者分别为52例(14.4%)、226例(62.8%)、82例(22.8%)。患者舒适度总均分为(66.83±10.02)分,中、低度舒适者分别为234例(65.0%)、126例(35.0%);患者睡眠质量总均分为(34.21±5.36)分,轻、中、重度睡眠问题者分别为63例(17.5%)、221例(61.4%)、63例(17.5%)。患者的疼痛程度、舒适度、鼻出血情况、过敏性鼻炎、鼻窦开窗个数是影响其睡眠质量的因素。患者的舒适度与睡眠质量呈正相关(P<0.05)。结论慢性鼻窦炎鼻内镜术后鼻腔填塞患者普遍存在中重度睡眠问题。要提高患者的睡眠质量,术后需加强疼痛管理,提高舒适度,减少出血量,重点关注过敏性鼻炎,鼻窦开窗个数多的患者。
Objective To investigate the sleep quality status and influencing factors of patients with nasal packing after endoscopic sinus surgery for chronic sinusitis.Methods A total of 360 patients with nasal packing after endoscopic sinus surgery for chronic sinusitis in the Department of Otolaryngology of the First Affiliated Hospital of University of South China from July 2020 to March 2021 were conveniently selected as the research objects.The survey tools were modified Kolcaba Comfort Scale,Self-rating Sleep Scale and pain numerical scale.Descriptive statistics and multiple linear regression were used for statistical analysis.Results The postoperative pain score was(5.34±1.54)points,with 52 cases(14.4%),226 cases(62.8%)and 82 cases(22.8%)suffering from mild,moderate and severe pain,respectively.The total score of patients′comfort was(66.83±10.02)points,among which 234 cases(65.0%)were moderately comfortable and 126 cases(35.0%)were moderately comfortable.The total sleep quality of patients was(34.21±5.36)points,and 63 cases(17.5%),221 cases(61.4%)and 63 cases(17.5%)had mild,moderate and severe sleep problems,respectively.Pain degree,comfort,nosebleeds,allergic rhinitis and the number of fenestrations of nasal sinuses were the factors that affect the sleep quality.There was a positive correlation between patient comfort and sleep quality(P<0.05).Conclusion Patients with nasal packing after endoscopic sinus surgery for chronic sinusitis generally have moderate to severe sleep problems.To improve the patients′sleep quality,it is necessary to strengthen the pain management,improve the comfort and reduce the amount of bleeding after operation,and pay special attention to patients with allergic rhinitis and a large number of sinus fenestrations.
