摘要
目的:观察加减甘草泻心汤对肺癌中晚期患者化疗后胃肠道反应的临床效果。方法:选取2019年1月-2020年12月长治市中医医院收治的肺癌中晚期化疗后胃肠道反应患者102例,按照随机数字表法分为对照组和观察组,各51例。对照组给予西药治疗,观察组则采用加减甘草泻心汤。对比两组患者胃肠道反应改善效果、中医证候评分、胃泌素水平、血清肿瘤标志物指标水平,生存及生活质量、不良反应发生率。结果:观察组胃肠道反应改善总有效率高于对照组(P<0.05);治疗前,两组胃痛、胃胀、呕吐、纳少等中医证候评分比较,差异无统计学意义(P>0.05),治疗后,两组中医证候评分均降低,但观察组明显低于对照组(P<0.05);治疗前,两组欧洲癌症研究治疗组织癌症患者生命质量问卷(Quality of Life Questionnaire for cancer patients,QLQ)-C30评分、生活质量Karnofsky功能状态评分法(Karnofsky Performance Status,KPS)评分差异均无统计意义(P>0.05),经治疗,两组QLQ-C30评分、KPS评分均有所提升,但观察组高于对照组(P<0.05);经治疗,观察组胃泌素、血清肿瘤标志物水平改善情况明显优于对照组(P<0.05),不良反应发生率低于对照组,P<0.05。结论:加减甘草泻心汤可明显改善肺癌中晚期患者化疗后胃肠道反应,提高患者的生存及生活质量,降低血清肿瘤标志物水平,且不良反应低,值得临床运用。
Objective:To observe the clinical effect of the Gancao Xiexin decoction(甘草泻心汤)on gastrointestinal reactions in patients with advanced lung cancer after chemotherapy.Methods:102 cases from January 2019 to December 2020 were selected,and were randomly divided into the control group and the observation group,with 51 cases in each group.The control group was treated with Western medicine,and the observation group was treated with the Gancao Xiexin decoction.Results:The total efficiency of ameliorating gastrointestinal reactions in the observation group was higher than that in the control group(P<0.05).After treatment,the scores of TCM syndromes such as stomach pain,bloating,vomiting,and anorexia in the two groups were decreased,and in the observation group was significantly lower than the control group(P<0.05).After treatment,the QLQ-C30 score and KPS score in the two groups were improved,and in the observation group was higher than the control group(P<0.05).After treatment,the improvement of gastrin and serum tumor marker levels in the observation group was significantly better than the control group(P<0.05),and the incidence of adverse reactions in the observation group was lower than the control group,P<0.05.Conclusion:The Gancao Xiexin decoction can significantly ameliorate the clinical symptoms of gastrointestinal reactions in patients with advanced lung cancer after chemotherapy,improve the survival and quality of life of patients,and decrease the level of serum tumor markers,with fewer adverse reactions.It is worthy of clinical application.
