摘要
目的:探讨肺癌社区管理模式对患者社会支持度和生活质量相关指标的影响。方法:选取南京市止马营社区医院等8个卫生机构的不住院肺癌患者200例。采用随机数字表法将其随机分为管理组100例、对照组100例,管理组采用"社会-心理-生物"三者综合管理模式,对照组不完全具备三者干预因素。采用领悟社会支持量表(PSSS)、欧洲癌症研究与治疗组织生命质量核心量表(EORTC QLQ-C30)和肺癌患者生活质量测定[FACT-L(V4.0)]比较两组患者干预前、干预6个月后、干预12个月后的评分情况。结果:管理组PSSS在干预12个月后有明显提高,与干预前和对照组同期相比差异有统计学意义(P<0.05)。干预6个月后,管理组EORTC QLQ-C30在情绪功能、认知功能方面分值高于对照组,差异有统计学意义(P <0.01);干预12个月后,管理组的功能领域中社会功能、情绪功能、认知功能与对照组比较差异有显著统计学意义(P <0.01)。症状领域中疲劳、疼痛、总体健康状况较对照组分值比较差异有统计学意义(P <0.05)。干预12个月后,管理组FACT-L(V4.0)在生理状况方面、情感状况方面、社会家庭状况方面及功能状况方面均有明显改善,与干预前和对照组同期相比差异有统计学意义(P <0.05)。结论:肺癌社区疾病管理模式能改善患者社会支持度,且干预越长、改善效果越佳。肺癌社区疾病管理模式能显著改善肺癌患者的生理状况、社会家庭状况、情感状况、功能状况,加快患者社会功能、情绪功能、认知功能的恢复,同时减轻疲劳、疼痛症状,且干预时间与改善效果呈正相关关系。
Objective:To investigate efficacy of lung cancer community management model on patients'social support and quality of life related indicators.Methods:200 cases of lung cancer with no hospitalized in 8 health institutions were randomly divided into the management group(100 cases)and control group(100 cases).The management group was given social-psychological-biological integrated management model,and the control group did not give complete intervention.Results:After 12 months of intervention,the PSSS in the management group was significantly improved,and there was statistically significant differences compared with the pre-intervention or the control group(P<0.05).After 6 months of intervention,the EORTC QLQ-C30 scores(emotional function,cognitive function)in the management group were higher than the control group(P<0.01).After 12 months of intervention,the functional area scores(social function,emotional function,cognitive function)in the management group were significantly different from the control group(P<0.01);the fatigue,pain and overall health status in the management group were significantly different from the control group(P<0.05).After 12 months of intervention,the FACT-L(V4.0)(physical,emotional,social family,function)in the management group was improved significantly(P<0.05).Conclusion:Lung cancer community disease management model can improve patients'social support;the intervention time is proportional to the improvement effect.The lung cancer community disease management model can significantly improve the physiological status,social family status,emotional status,and functional status of lung cancer patients,promote the recovery of social functions,emotional functions,and cognitive functions,reduce fatigue and pain symptoms.
