摘要
盆腔器官脱垂(POP)是女性常见的盆底功能障碍,老年人群的患病率逐年升高,手术是POP的有效治疗方法,而聚丙烯网片是其最重要的手术材料。细胞外基质(ECM)具有减轻异物炎症反应、促进新生胶原成熟、维持平滑肌形态与功能的作用,具有ECM涂层聚丙烯网既保留了传统网片的机械强度,又可减轻传统网片因异物反应导致的炎症。为了适应POP手术的临床需求,现已开发的组织原位脱细胞法能够防止盆底组织塌陷,并提供成像系统,通过优化电纺丝制造技术获得的ECM网片3D结构更加贴合盆底结构,且在聚丙烯网片中添加脂溶性雌二醇可刺激盆底血管再生,有助于盆底重建术后恢复。因此,ECM涂层聚丙烯网在女性盆底手术中有较好的应用前景。
Pelvic organ prolapse(POP),as a common pelvic floor dysfunction,has an increasing prevalence in elderly women.Surgery is an effective method for the treatment of POP,and the most important surgical material is polypropylene mesh.Extracellular matrix(ECM)can reduce foreign body inflammatory reaction,promote the maturation of new collagen and maintain the morphology and function of smooth muscle.ECM coated polypropylene mesh not only retains the mechanical strength of traditional mesh,but also reduces the inflammation caused by foreign body reaction caused by traditional mesh.In order to meet the clinical needs of pelvic organ prolapse surgery,in situ decellularization of tissues acellular technology has been developed to prevent pelvic floor tissue collapse and provide imaging system,and the 3D structure of ECM mesh obtained from the optimized electrospinning manufacturing technology fits the basin bottom structure better,while adding fat soluble estradiol can stimulate pelvic floor vascular regeneration,which is helpful to the recovery after pelvic floor reconstruction.Therefore,ECM coated polypropylene mesh has a good applicative prospect in female pelvic floor surgery.
