摘要
先兆流产是常见的妊娠合并症,子宫内血肿是先兆流产的并发症之一,是导致流产的高危因素,研究发现子宫内血肿会使流产、早产、宫内生长受限、胎盘早剥、低出生体质量、新生儿窒息等不良妊娠结局的风险增高。先兆流产合并子宫内血肿临床并不少见,也是对母婴造成很大危害的妇产科常见病,随着工作压力的逐渐增大、生活饮食习惯的改变及三胎生育政策的开放,本病的发生有着逐年上升的趋势,目前其发病因素及形成机制尚不完全明确,且国内外尚无统一治疗规范及指南。韩氏认为其中医病机在于肾气不固、瘀血内阻于冲任胞宫、胎元失养失固所致,治疗当补肾安胎为主,活血祛瘀为辅,方用韩氏滋肾汤,并随症加减,其临床疗效显著,并附典型医案一则以资佐证。
Threatened abortion is a common pregnancy complication.Intrauterine hematoma is one of the complications of threatened abortion and one of the high-risk factors leading to abortion.It is found that intrauterine hematoma will increase the risk of adverse pregnancy outcomes such as abortion,premature birth,intrauterine growth restriction,placental abruption,low birth weight and neonatal asphyxia.Threatened abortion combined with intrauterine hematoma is not uncommon in clinic.It is also a common disease in obstetrics and gynecology that causes great harm to mothers and infants.With the gradual increase of work pressure,the change of living and eating habits and the opening of the three child fertility policy,the incidence of this disease is increasing year by year.At present,its pathogenesis and formation mechanism are not completely clear,And there are no unified treatment norms and guidelines at home and abroad.HAN believes that the medical pathogenesis is that the kidney Qi is not solid,the blood stasis is internally blocked in the Chongren uterus,and the fetal element is dystrophic and solid.The treatment should be mainly for invigorating the kidney and anti-fetus,supplemented by promoting blood circulation and removing blood stasis.The prescription is HAN's Zishen Decoction,which is added or subtracted according to the symptoms.The clinical effect is remarkable.Therefore,a typical medical case was attached for evidence.
