摘要
目的 观察复方氯己定含漱液联合氟康唑治疗口腔念珠菌病的临床效果,并观察其对患者症状改善情况及复发率的影响。方法 选取2019年8月—2020年9月泉州市第一医院收治的口腔念珠菌病患者78例为研究对象,依据随机数字表法分为联合组和单一组,每组39例。单一组实施聚维酮碘含漱液治疗,联合组实施复方氯己定含漱液联合氟康唑治疗,2组均连续治疗14 d。比较2组治疗总有效率,治疗前后症状积分改善情况,不良反应及疾病复发率。结果 治疗14 d,联合组总有效率为94.87%,高于单一组的76.92%(χ^(2)=5.186,P=0.023);2组口腔疼痛评分、红斑评分均较治疗前下降,且联合组低于单一组(P均<0.01);联合组不良反应总发生率为7.69%,低于单一组的25.64%(χ^(2)=4.523,P=0.033);随访3个月,联合组疾病复发率为10.26%,低于单一组的38.46%(χ^(2)=8.419,P=0.004)。结论 复方氯己定含漱液联合氟康唑治疗口腔念珠菌病效果确切,可显著改善患者的症状,不良反应少,安全性较高,且可降低疾病复发率,值得临床推广应用。
Objective Observe the clinical effect of compound chlorhexidine gargle combined with fluconazole in patients with oral candidiasis,and observe its effect on the improvement of symptoms and recurrence rate. Methods 78patients with oral candidiasis admitted to Quanzhou First Hospital from August 2019 to September 2020 were selected as the research objects. They were divided into combined group and single group according to the random number table method,with39 cases in each group. The single group was treated with povidone iodine gargle,and the combined group was treated with compound chlorhexidine gargle combined with fluconazole. Both groups were treated for 14 days. Compare the total effective rate of the two groups of treatment,the improvement of the symptom score before and after the treatment,the adverse reaction and the disease recurrence rate. Results After 14 days of treatment,the total effective rate of the combined group was94. 87%,which was higher than 76. 92% of the single group( χ^(2)= 5. 186,P = 0. 023);The oral pain score and erythema score of the two groups were lower than before treatment,and the combined group was lower than the single group( all P <0. 01);The total incidence of adverse reactions in the combined group was 7. 69%,which was lower than 25. 64% in the single group( χ^(2)= 4. 523,P = 0. 033);After 3 months of follow-up,the disease recurrence rate in the combined group was10. 26%,which was lower than 38. 46% in the single group( χ^(2)= 8. 419,P = 0. 004). Conclusion The compound chlorhexidine gargle combined with fluconazole has a definite effect in the treatment of oral candidiasis. It can significantly improve the symptoms of patients,has fewer adverse reactions,higher safety,and can reduce the recurrence rate of the disease. It is worthy of clinical application.
