摘要
目的:观察补中益气汤加减联合氟康唑治疗白色念珠菌性肺炎的临床疗效。方法:选取78例白色念珠菌性肺炎患者,按随机数字表法分为观察组和对照组各39例。对照组给予氟康唑治疗,观察组在对照组基础上给予补中益气汤加减治疗。比较2组临床疗效、症状评分、(1,3)-β-D-葡聚糖水平及痰真菌培养转阴率。结果:观察组总有效率(97.44%)高于对照组(84.62%),差异有统计学意义(χ_(2)=3.924,P=0.048)。治疗后,2组咳嗽、喘息、咳痰评分均低于治疗前(P<0.05);观察组治疗后咳嗽、喘息、咳痰评分低于对照组(P<0.05)。治疗3 d、7 d、10 d、14 d,2组(1,3)-β-D-葡聚糖水平较治疗前均呈不断下降趋势(P<0.05),观察组各时间点(1,3)-β-D-葡聚糖水平均低于对照组(P<0.05)。观察组痰真菌转阴率(89.74%)高于对照组(71.79%),差异有统计学意义(χ_(2)=4.044,P=0.044)。结论:补中益气汤加减联合氟康唑治疗白色念珠菌性肺炎效果显著,可有效缓解临床症状,降低(1,3)-β-D-葡聚糖水平,提高痰真菌培养转阴率。
Objective:To observe the clinical effect of the therapy of modified Buzhong Yiqi tang combined with fluconazole on candida albicans pneumonia.Methods:A total of 78 patients with candida albicans pneumonia were selected and divided into the observation group and the control group according to the random number table method,with 39 cases in each group.The control group was treated with fluconazole,and the observation group was additionally treated with modified Buzhong Yiqi tang based on the treatment of the control group.The clinical effects,symptom scores,levels of(1,3)-β-Dglucan and negative conversion rates of sputum fungal culture in the two groups were compared.Results:The total effective rate was 97.44%in the observation group,higher than that of 84.62%in the control group,the difference being significant(χ_(2)=3.924,P=0.048).After treatment,the scores of cough,wheezing and expectoration in the two groups were lower than those before treatment(P<0.05);after treatment,the above scores in the observation group were lower than those in the control group(P<0.05).On the 3 rd,7 th,10 thand 14 thdays during treatment,the levels of(1,3)-β-D-glucan in both groups showed a continuously decreasing trend when compared with those before treatment(P<0.05),and the above level at each time point in the observation group was lower than that in the control group respectively(P<0.05).The negative conversion rate of sputum fungal culture was 89.74%in the observation group,higher than that of 71.79%in the control group,the difference being significant(χ_(2)=4.044,P=0.044).Conclusion:The therapy of modified Buzhong Yiqi tang combined with fluconazole has a remarkable effect on candida albicans pneumonia,which can effectively relieve the clinical symptoms,reduce the level of(1,3)-β-D-glucan,and improve the negative conversion rate of sputum fungal culture.
