摘要
目的探究超脉冲CO_(2)点阵激光治疗烧伤后面部色素沉着的效果。方法自2018年6月至2020年6月,宝鸡市中心医院对80例烧伤后面部色素沉着的患者,采用随机数字表法进行分组,每组40例。对照组采用强脉冲光治疗,观察组采用超脉冲CO_(2)点阵激光治疗,两组治疗后均涂抹重组牛碱性成纤维细胞生长因子凝胶,2 d后七子白中药面膜外敷。所有患者分别于治疗前、治疗后1、2、3个月测定和比较两组面部图像评分(色素沉着、紫质)、皮肤性质评分(水分、弹性)、氧化应激指标(丙二醛、超氧化物歧化酶、过氧化脂质)变化情况,对比总有效率的差异。结果总有效率:观察组为95.00%(38/40),对照组为85.00%(34/40),两组差异有统计学意义(P<0.05)。所有患者均无不良反应发生。两组患者面部图像评分,皮肤性质评分,氧化应激指标治疗后1、2、3个月较治疗前明显改善,且观察组各时间段指标数值优于对照组,差异有统计学意义(P<0.05)。结论超脉冲CO_(2)点阵激光治疗烧伤后面部色素沉着效果确切,可以有效地抑制局部组织氧化应激反应,值得临床推广使用。
Objective To study the efficacy of ultra-pulse CO_(2) fractional laser in the treatment of facial pigmentation after burns.Methods From June 2018 to 2020, 80 patients with facial pigmentation after burns were randomly divided into 2 groups, 40 cases in each group. The control group was treated by intense pulsed light and the observation group was treated by ultra-pulsed CO_(2) fractional laser. The recombinant bovine basic fibroblast growth factor gel was performed in both groups after treatment and changed to traditional Chinese medicine mask of "Qibai San" at 2 days after treatment. The changes of facial image scores(pigmentation and purpurin), skin property scores(moisture and elasticity) and oxidative stress indicators(malondialdehyde, superoxide dismutase and lipid peroxidase) before treatment and at 1 month, 2 months and 3 months after treatment were detected and compared. The difference in total effective rate was compared. Results The total effective rate was 95.00%(38/40) in the observation group and 85.00%(34/40) in the control group, the difference was statistically significant(P<0.05). No adverse reactions were found in all patients. The scores of facial image, skin property and oxidative stress indicators ofboth groups at 1 month, 2 months and 3 months after treatment were significantly improved. The index values in observation group were better than those in control group at each period, the difference was statistically significant(P<0.05). Conclusion Ultra-pulse CO_(2) fractional laser has a definite efficacy in the treatment of facial pigmentation after burns. This method can effectively inhibit the oxidative stress response of local tissues. It should be considerable for clinical popularization.
