实时剪切波弹性成像在前列腺癌诊断及风险分级中的应用价值

摘要 目的探讨经直肠实时剪切波弹性成像技术(SWE)在前列腺癌(PCa)诊断及风险评估中的应用价值。方法选取2015年6月至2020年12月因尿频或排尿困难等症状就诊于宝鸡市中心医院的349例患者,其在行前列腺穿刺活检前均接受了直肠SWE检查,在6个位置测量前列腺的杨氏模量,以测量结果的平均值作为前列腺标准化硬度,包括Emax、Emean及Emin。比较不同性质前列腺疾病患者的临床资料与标准化硬度;再将PCa患者分为低危组、中危组及高危组,比较各组的前列腺标准化硬度值;分析前列腺标准化硬度对高危PCa的诊断效能。结果恶性组的前列腺标准化硬度Emax、Emean及Emin均大于良性组,差异具有统计学意义(P<0.05)。高危组的前列腺标准化硬度Emax、Emean及Emin均大于中危组及低危组,差异具有统计学意义(P<0.05);中危组与低危组的前列腺标准化硬度Emax、Emean及Emin比较,差异无统计学意义(P>0.05)。Emax与Emean的AUC均大于Emin,差异具有统计学意义(P<0.05);Emax与Emean的AUC比较,差异无统计学意义(P>0.05)。结论 SWE测量的前列腺标准化硬度对PCa的诊断具有一定的参考价值,并可能成为检测高危PCa的一种简便而可靠的超声影像学方法。 Objective To explore the application value of transrectal real-time shear wave elastography(SWE) in the diagnosis and risk classification of prostate cancer(PCa).Methods A total of 349 patients admitted in Baoji central hospital from June 2015 to December 2020 due to frequent urination or dysuria were selected,and all underwent transrectal SWE examination before prostate biopsy.The Young’s modulus of the prostate was measured in six locations,the average of the measurement results was defined as the standardized stiffness of prostate gland,including Emax,Emean and Emin.The clinical data and standardized stiffness of patients with different nature of prostate disease were compared;the PCa patients were divided into low-risk,medium-risk and high-risk groups,and the standardized stiffness of prostate gland of each group were compared;the diagnostic efficacy of standardized stiffness of prostate gland for high-risk PCa was analyzed.Results The Emax,Emean and Emin of standardized stiffness of prostate gland in the malignant group were significantly higher than those in the benign group,and the differences were statistically significant(P<0.05).The Emax,Emean and Emin of standardized stiffness of prostate gland in the high-risk group were higher than those in the medium-risk and the low-risk groups,and the differences were statistically significant(P<0.05);there were no significant differences in Emax,Emean and Emin of standardized stiffness of prostate gland between the medium-risk and the low-risk groups(P>0.05).The AUC of Emax and Emean was greater than that of Emin,and the differences were statistically significant(P<0.05);there was no significant difference in AUC between Emax and Emean(P>0.05).Conclusion The standardized stiffness of prostate gland measured by SWE has certain reference value in the diagnosis of PCa,and it may be a simple and reliable ultrasonic imaging method for high-risk PCa detection.
