摘要
目的观察七宝美髯汤加减联合非剥脱性点阵铒激光治疗雄激素性脱发(AGA)的临床疗效及对头皮血流动力学和毛囊的影响。方法将96例AGA患者按照随机数字表法分为2组。对照组48例予非剥脱性点阵铒激光治疗,治疗组48例在对照组治疗基础上联合七宝美髯汤加减治疗。2组均治疗6周。比较2组治疗前后临床症状评分、脱发区头皮血流动力学指标[动脉内径、峰值流速(V_(max))、血流阻力指数(RI)、血流速度时间积分(VTI)]、毛囊数量及治疗后毛发生长情况(毛发生长速度、新生数量),并统计2组疗效及不良反应发生情况。结果治疗组总有效率95.8%(46/48),对照组总有效率83.3%(40/48),治疗组疗效优于对照组(P<0.05)。治疗后2组各症状评分及总分均较本组治疗前降低(P<0.05),且治疗后治疗组均低于对照组(P<0.05)。治疗后治疗组动脉内径、V_(max)、VTI均较本组治疗前升高(P<0.05),RI降低(P<0.05),对照组动脉内径、VTI均升高(P<0.05),RI降低(P<0.05),且治疗后治疗组动脉内径、VTI均高于对照组(P<0.05),RI低于对照组(P<0.05)。治疗后2组毛囊数量均较本组治疗前升高(P<0.05),且治疗后治疗组毛囊数量、毛发生长速度、毛发新生数量均高于对照组(P<0.05)。结论七宝美髯汤加减联合非剥脱性点阵铒激光治疗AGA疗效确切,可以改善头皮血流动力学指标,有效促进毛囊和毛发生长,值得临床推广应用。
Objective To observe the clinical efficacy of non-ablative fractional erbium laser with modified Qibao Meiran Decoction for androgenetic alopecia(AGA)and how it impacts on hemodynamic and hair follicle.Methods Totally 96 patients with AGA were divided into equal group according to the random number table,in addition to the non-ablative fractional erbium laser,and treatment group additionally received modified Qibao Meiran Decoction.Treated for 6 weeks,the aim was to compare clinical score,hemodynamic indices of scalp hair in bald area[artery diameter,peak systolic velocity(V_(max)),blood flow resistance index(RI),velocity time integral(VTI)],hair follicle quantity and hair growth(hair growth rate,quantity);the curative effect and adverse reactions were counted.Results The total effective rates were better in treatment group than in control group[95.8%(46/48)vs 83.3%(40/48),(P<0.05),respectively].After treatment,clinical scores in groups were decreased(P<0.05),treatment group was lower as compared with control group(P<0.05);artery diameter,V_(max) and VTI in treatment group were increased than before treatment(P<0.05),RI was decreased(P<0.05);artery diameter and VTI in control group were increased(P<0.05),RI was decreased(P<0.05);artery diameter and VTI were higher in treatment group than in control group(P<0.05),while RI was lower than control group(P<0.05);hair follicle quantity in groups was increased(P<0.05),hair follicle quantity,hair growth rate and quantity of new hair were increased in treatment group compared with cortrol group(P<0.05).Conclusion For patients with AGA,non-ablative fractional erbium laser with modified Qibao Meiran Decoction has a definite curative effect,which is effectively capable of improving hemodynamic indexes of scalp,promoting hair follicle and hair growth,and worthy of clinical application.
