摘要
目的探究原发性干燥综合征(primary Sjgren′s syndrome, pSS)患者与继发性干燥综合征(secondary Sjgren′s syndrome, sSS)患者的肠道菌群差异及其与临床免疫指标之间的相关性。方法对15例pSS患者和10例sSS患者进行临床数据统计、血清标本和粪便标本采集。将采集到的粪便标本进行DNA提取和筛选,再经16S rDNA高通量测序后进行OTU分析,Alpha、Beta多样性以及样品组间显著性差异分析。同时使用免疫荧光法检测自身抗体ANA,免疫印迹法检测抗核抗体谱(SSA、SSB、Ro52),散射比浊法检测风湿系列(RF、CRP)、免疫球蛋白(IgG等)和补体(C3、C4)等12种免疫学指标,并分析肠道菌群丰度与免疫指标浓度的相关性。结果 pSS患者与sSS患者肠道菌群存在差异,pSS患者的肠道菌群多样性大于sSS患者。在门水平上,sSS患者较pSS患者厚壁菌门丰度降低、变形菌门丰度升高。经t-test检验,在科水平,链球菌科为2组差异最为显著的菌科(t=0.029,P<0.01),同时在属水平,链球菌属也是差异最大的菌属(t=0.029,P<0.01),CRP与Parabacteroides等多个菌属有显著的相关性。结论 pSS患者与sSS患者肠道菌群存在明显差异,这些差异可能与SSA、SSB和CRP具有相关性。
Objective To explore the features and differences of intestinal flora and their associations with some laboratory markers in patients with primary Sjgren′s syndrome(pSS) vs secondary Sjgren′s syndrome(sSS). Methods Clinical data as well as serum and stool samples were obtained from 15 pSS patients and 10 sSS patients. Total metagenomic DNA of each fecal sample was extracted, checked and analyzed by using 16 S rDNA high-throughput sequencing. Data analysis were subsequently performed, including OTU analysis, Alpha and Beta diversities analysis and significant difference analysis between two groups. Additionally, autoantibody ANA was detected with immunofluorescence, antinuclear antibody spectrum(SSA, SSB, Ro52) was detected with immunoblotting, and the immunological indexes such as rheumatic series(RF, CRP), immunoglobulin(IgM, etc) and complement(C3, C4) were detected with scatter turbidimetry. We also analyzed the correlations between intestinal flora abundances and the levels of immune indexes. Results There were differences in intestinal flora between patients with pSS and those with sSS. The diversity of intestinal microflora in patients with pSS was higher than that in patients with sSS. At the phylum level, sSS patients had lower level of Firmicutes and higher level of Proteobacteria than those with pSS. According to the t-test test, at the family level, Streptococcaceae had the most significant difference between the two groups(t=0.029, P<0.01), and at the genus level, Streptococcus was also the most different bacterial genus(t=0.029, P<0.01), furthermore, there was significant correlations between CRP and many genera such as Parabacteroides. Conclusion There are significant differences in intestinal flora between pSS patients and sSS patients, which may be related to the levels of SSA, SSB and CRP.
