摘要 目的检测高迁移率族蛋白B1(high mobility group protein B1,HMGB1)、白细胞介素-6(interleakin-6,IL-6)在分泌性中耳炎患者血清及耳积液表达水平,探讨其在分泌性中耳炎中的致病意义。方法选择2019年6月~2020年12月本院治疗的分泌性中耳炎患者83例,依据病情分期分为急性组(急性中耳炎,病程<3周)28例、亚急性组(亚急性中耳炎,病程3周~3个月)30例和慢性组(慢性中耳炎,病程3~6个月)25例;根据耳积液性质分为浆液性组50例、黏液性组33例,另选取同期健康体检人员(无任何耳鼻喉疾病史)抽血并留血清85例作为对照组,采用ELISA方法检测血清及耳积液HMGB1、IL-6水平,采用Pearson法分析HMGB1、IL-6水平与分泌性中耳炎患者病程的相关性。结果对照组与观察组血清HMGB1、IL-6水平差异无统计学意义(P>0.05);与观察组血清比较,分泌性中耳炎患者耳积液HMGB1、IL-6水平显著升高(P<0.05);急性组、亚急性组、慢性组中耳炎患者耳积液HMGB1、IL-6表达水平依次升高,差异有统计学意义(P<0.05);与浆液性组比较,黏液性组患者耳积液HMGB1、IL-6表达水平显著升高,(P<0.05);相关性分析显示,分泌性中耳炎患者耳积液HMGB1、IL-6表达水平与患者病程呈正相关(r=0.667,r=0.546,P<0.05)。结论HMGB1、IL-6在分泌性中耳炎患者耳积液中呈明显高表达,与分泌性中耳炎病程及炎症反应程度有关,其可能参与分泌性中耳炎的发生发展。 Objective To detect the expression levels of high mobility group protein B1(HMGB1)and interleukin-6(IL-6)in serum and ear effusion of patients with secretory otitis media,and to explore the significance of HMGB1 and IL-6 in secretory otitis media.Methods Eighty-three patients with secretory otitis media treated in our hospital from June 2019 to December 2020 were selected;they were divided into acute group(acute otitis media,course less than 3 weeks)with 28 cases,subacute group(subacute otitis media,course 3 weeks to 3 months)with 30 cases and chronic group(chronic otitis media,course 3 to 6 months)with 25 cases according to the stage of the patient’s condition;they were divided into serous group(n=50)and mucinous group(n=33)according to the nature of the effusion.Another 85 healthy people(without any history of otorhinolaryngology)were selected as the control group,who were drawn blood and collected the serum.The levels of HMGB1 and IL-6 in serum and ear effusion were detected by ELISA.The correlation between HMGB1,IL-6 levels and the course of secretory otitis media were analyzed by Pearson method.Results There was no significant difference in serum HMGB1 and IL-6 levels between the control group and the observation group(P>0.05);Compared with those in the serum of the observation group,the levels of HMGB1 and IL-6 in ear effusion in the patients with secretory otitis media were significantly increased,and the differences were statistically significant(P<0.05).The expression levels of HMGB1 and IL-6 in ear effusion of patients in the acute otitis media group,the subacute otitis media group and the chronic otitis media group increased in turn,and the differences were statistically significant(P<0.05);Compared with those in serous group,the expression levels of HMGB1 and IL-6 in ear effusion in the mucinous group were significantly higher,the differences were statistically significant(P<0.05);Correlation analysis showed that the expression levels of HMGB1 and IL-6 were positively correlated with the course of d
