摘要
目的考察严重主动脉瓣狭窄(severe aortic stenosis,SAS)患者的主动脉瓣流量(aortic valve stroke volume,AVSV)、主动脉瓣狭窄程度与全身耗氧量(systemic oxygen consumption,SOC)的相关性。方法2015年12月至2018年11月,年龄≥18岁的患者被诊断为≥轻度AS[主动脉瓣钙化,受限制的收缩运动和主动脉瓣口面积(AVA)<2 cm^(2)]和LVEF≥50%的患者被前瞻性鉴定并包含在电子数据库中。根据其基线压力梯度TPG将患者回顾性分为3组:第1组为TPG≥40 mm Hg的严重主动脉瓣狭窄患者(n=100);第2组为TPG<40 mm Hg的主动脉瓣狭窄患者(n=100);第3组为对照组(n=100,无主动脉瓣狭窄者)。采集患者的超声造影动态图像计算AVSV,Datex呼出气体监测仪器进行SOC测定。将得到的AVSV、TPG结果与测得的SOC进行Pearson相关性分析。结果严重主动脉瓣狭窄患者的主动脉瓣流量、主动脉瓣狭窄程度与全身耗氧量呈负相关(P<0.05);严重主动脉瓣狭窄患者的主动脉瓣狭窄程度与主动脉瓣流量呈正相关(P<0.05)。结论全身耗氧量评估可以成为严重主动脉瓣狭窄患者治疗决策过程的一个重要因素。
Objective To investigate the correlation between aortic valve stroke volume(AVSV),the aortic stenosis degree and systemic oxygen consumption(SOC)in patients with severe aortic stenosis(SAS).Methods A total of 300 patients aged≥18 years who were diagnosed as mild AS(aortic valve calcification,restricted contractile movement and AVA<2cm^(2))and LVEF≥50%were prospectively identified and included in an electronic database from December 2015 and November 2018.The patients were retrospectively divided into three groups according to baseline pressure gradient TPG:group 1 with TPG≥40mmHg(n=100),group 2 with TPG≤40mmHg(n=100),group 3 was control group(without aortic stenosis),with 100 cases in each group.The dynamic images by ultrasound visualization were collected to calculate AVSV,and Datex expired gas monitor was used to measure SOC,the correlation between SOC and AVSV,TPG was analyzed by Pearson correlation analysis.Results The AVSV and the aortic stenosis degree were negatively correlated with SOC in patients with SAS(P<0.05).However the aortic stenosis degree was positively correlated with AVSV(P<0.05).Conclusion The systemic oxygen consumption assessment can be regarded as an important factor in making the treatment scheme for the patients with SAS.
