摘要
目的:研究CHA_(2)DS_(2)-VAS_(C)评分与非房颤合并脑梗死急性期神经功能和预后的关系。方法:根据CHA_(2)DS_(2)-VAS_(C)评分将180例非房颤合并脑梗死患者分为三组,美国国立卫生研究院卒中量表(NIHSS)评分对患者进行神经功能评分,比较患者NIHSS评分,随访6个月时病残率及复发率。结果:180例脑梗死患者中低危组32例、中危组35例、高危组113例。高危组神经功能缺损最严重[NIHSS评分(18.96±7.83)],其次是中危组[NIHSS评分(7.86±3.42)]、低危组[NIHSS评分4.61±2.15)],差异有统计学意义(均P<0.05)。高危组随访6个月时病残率38.05%、复发率28.32%,与中危组(病残率20.00%、复发率25.71%)及低危组(病残率3.13%、复发率6.25%)相比较,差异有统计学意义(均P<0.05)。结论:CHA_(2)DS_(2)-VAS_(C)评分可以用来评估非房颤合并脑梗死急性期神经功能和预后。
Objective:To study the relationship of CHA_(2)DS_(2)-VAS_(C) score with acute neurological impairment and prognosis in cerebral infarction patients without atrial fibrillation.Methods:180 cerebral infarction patients were classified into three groups.National Institue of Health Stroke Scale(NIHSS)was used to evaluate acute neurological impairment at admission.NIHSS score,6-month disability rate and 6-month stroke recurrence rate in three groups were compared.Results:In 180 cerebral infarction patients,there were 32 cases(17.78%)in low risk group,35 cases(19.44%)in medium risk group,113 cases(62.78%)in high risk group.The neurological impairment in high risk group was most severe[NIHSS score(18.96±7.83)],the next was medium risk group[NIHSS score(7.86±3.42)].The neurological impairment in low risk group was most mild[NIHSS score(4.61±2.15)].The differences were significant(P<0.05,for all).The 6-month disability rate[38.05%(43/113)]and 6-month stroke recurrence rate[28.32%(32/113)]in high risk group were higher than those in medium risk group[20.00%(7/35),25.71%(9/35),respectively]and those in low risk group[3.13%(1/32),6.25%(2/32),respectively].The differences were significant(P<0.05,for all).Conclusion:CHA_(2)DS_(2)-VAS_(C) score can be a useful predictive index for acute neurological degree and prognosis in cerebral infarction patients without atrial fibrillation.
