摘要
本研究基于2017年修订版(第三版)JOHNS HOPKINS护理循证实践模型开展研究。首先组建循证护理小组,检索、评价并汇总最佳证据,确定审查指标及审查方法,随后进行基线审查。根据基线审查结果分析循证实践的障碍因素和推动因素,制定循证实践策略。本研究共纳入11条最佳证据,根据最佳证据制定了16条审查指标。此外,本研究发现障碍因素为护士缺乏高龄经产妇产后循证护理相关知识;缺乏对高龄经产妇产后优化护理管理的意识;年资较高的护士习惯传统护理措施,缺乏循证实践知识及接受临床变革依从性差。因此,临床上需针对高龄孕产妇优化护理的临床实践,客观并充分地考虑实施过程中的障碍因素,有针对地制定策略,进而在临床开展循证实践,改善高龄经产妇产后护理流程,并在临床中持续应用及改进。
This study was conducted based on the 2017 revised(third edition)JOHNS HOPKINS model of evidence-based nursing practice.An evidence-based nursing team was first formed to retrieve,evaluate and summarize the best evidence,identify review indicators and review methods,and subsequently conduct a baseline review.Barriers and facilitators to evidence-based practice were analyzed based on the results of the baseline review,and evidence-based practice strategies were developed.A total of 11 best evidence items were included in this study,and 16 review indicators were developed based on the best evidence.In addition,the barrier factors identified in this study were nurses’lack of knowledge related to evidence-based postpartum care for older multiparas,lack of awareness of optimal postpartum care management for older multiparas,and nurses with higher seniority who were accustomed to traditional care practices and lacked knowledge of evidence-based practice and adherence to clinical change.Therefore,the clinical practice of optimal care for advanced maternal age needs to be addressed by objectively and adequately considering the barriers to implementation,developing strategies correspondingly,and then carrying out evidence-based practice in the clinic to improve the process of postpartum care for older multiparas,and continuously applying and improving it in the clinic.
