摘要
目的研究高龄经产妇与妊娠并发症的关系。方法检索2000年1月1日至2020年12月20日中国知网、维普、万方数据库,选择国内有关高龄经产妇与妊娠并发症关系的文献,使用RevMan 5.3软件进行Meta分析。结果按照筛选标准共纳入15篇文献,合计高龄经产妇8 248例,非高龄经产妇16 500例。Meta分析结果显示高龄经产妇妊娠期高血压疾病(OR=2.54,95%CI:1.88~3.43)、妊娠期糖尿病(OR=2.00,95%CI:1.72~2.32)、前置胎盘(OR=1.60,95%CI:1.17~2.18)、胎膜早破(OR=1.58,95%CI:1.26~1.97)、产后出血(OR=1.54,95%CI:1.23~1.93)的发生风险更高,而胎盘早剥(OR=1.33,95%CI:0.74~2.40)、羊水量异常(OR=1.08,95%CI:0.80~1.44)、早产(OR=1.21,95%CI:0.92~1.59)和高龄经产妇无显著相关性。结论目前国内高龄经产妇与妊娠期高血压疾病、妊娠期糖尿病、前置胎盘、胎膜早破、产后出血的发生有一定关系,需引起重视。
Objective To study the relationship between elderly multiparas and pregnancy complications.Methods To search the databases of CNKI,VIP and Wanfang from 1 January 2000 to 20 December 2020,and select the domestic literatures about the relationship between elderly multiparas and pregnancy complications.Meta-analysis was performed using RevMan 5.3 software.Results According to the screening criteria, a total of 15 studies were included, with a total of 8 248 elderly multiparas and 16 500 non-elderly multiparas.Meta-analysis results showed that the risks of gestational hypertension(OR=2.54,95%CI:1.88-3.43),gestational diabetes(OR=2.00,95%CI:1.72-2.32),placenta previa(OR=1.60,95%CI:1.17-2.18),premature rupture of membranes(OR=1.58,95%CI:1.26-1.97),and postpartum hemorrhage(OR=1.54,95%CI:1.23-1.93) were significantly higher in elderly multiparas, while there were no significant correlations between the risks of placental abruption(OR=1.33,95%CI:0.74-2.40),abnormal amniotic fluid volume(OR=1.08,95%CI:0.80-1.44),and premature delivery(OR=1.21,95%CI:0.92-1.59) and elderly multiparas.Conclusion At present, there is a certain relationship between elderly multiparas and the occurrence of gestational hypertension, gestational diabetes, placenta previa, premature rupture of membranes, and postpartum hemorrhage, which should be paid more attention.
