摘要
目的探讨不同体重指数对多囊卵巢综合征(PCOS)患者促排卵治疗妊娠结局的影响。方法选取2018年1月至2020年1月在廊坊市人民医院治疗的295例患者,根据体重指数分为低体重组(n=20)、正常体重组(n=120)、超重组(n=40)、肥胖组(n=115),给予来曲唑联合周期人绝经期促性腺激素(HMG)促排卵治疗,比较4组样本一般资料、激素水平[卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、孕激素(P)、抗苗勒氏管激素(AMH)、睾酮(T)]、IR值、排卵率、临床妊娠率、生化妊娠率以及早期自然流产率。结果超重组、肥胖组与正常体重组比较,随着体重指数增加,LH与FSH比值减小,T和IR也随之增加,差异有统计学意义(P<0.05)。肥胖组、超重组临床妊娠率低于正常体重组,而生化妊娠率和自然流产率均高于正常体重组,差异均有统计学意义(P<0.05)。Logistic回归分析显示:BMI是影响PCOS患者自然流产的独立危险因素[OR=1.08,95%CI(1.01,1.17),P<0.05]。结论对于肥胖PCOS患者助孕前通过调整生活方式、控制饮食、适当运动等方式科学减重有益于改善促排卵妊娠的结局。
Objective To investigate the effects of body mass index(BMI)on the outcome of ovulation induction in patients with polycystic ovary syndrome(PCOS).Methods A total of 295 patients with PCOS who were treated in our hospital from January 2018 to January 2020 were enroolled in the tusy,who were randomly divided into underweight group(n=20),normal group(n=120),overweight group(n=40)and obese group(n=115)according to BMI.All patients were treated with Letrozole combined with HMG.The general data,hormone levels,IR value,ovulation rate,clinical pregnancy rate,biochemical pregnancy rate and abortion rate were compared between the four groups.Results Compared with those in normal group,the LH/FSH ratio decreased while T and IR value increased with the increase of BMI in overweight group and obese group(P<0.05).Moreover,the clinical pregnancy rates of the patients in overweight group and obese group were lower than that in normal group while the biochemical pregnancy rate and abortion rate were higher than that in normal group(P<0.05).Logistic regression analysis showed that BMI was an independent risk factor for spontaneous abortion in PCOS patients(P<0.05).Conclusion Weight loss in obese patients with PCOS by adjusting their lifestyle,controlling diet and exercising properly helps to improve the outcome of ovulatory pregnancy.
