摘要
目的探讨血清细胞色素C(Cyt-C)、高迁移率族蛋白B1(HMGB1)、Toll样受体4(TLR4)联合检测对新生儿早发型败血症(EOS)的预测价值。方法选取我院2019年4月至2021年4月收治的EOS患儿97例作为EOS组,选取同期于我院产科分娩的单纯细菌感染新生儿80例及健康新生儿82例分别为细菌感染组、对照组。比较3组血清Cyt-C、HMGB1、TLR4水平及降钙素原(PCT)、超敏C反应蛋白(hs-CRP)、白介素-6(IL-6)、总胆红素(TBIL)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、肌酸激酶同工酶(CK-MB)、肌钙蛋白I(cTnI)。采用Pearson线性相关分析血清Cyt-C、HMGB1、TLR4水平与炎症因子、肝功能、心肌受损指标的相关性以及Cyt-C与HMGB1、TLR4的相关性。利用受试者工作特征曲线(ROC)分析三者预测EOS的曲线下面积(AUC)。结果EOS组血清Cyt-C、HMGB1、TLR4、PCT、hs-CRP、IL-6、TBIL、ALT、AST、CK-MB、cTnI水平高于细菌感染组与对照组(P<0.05)。重症组血清Cyt-C、HMGB1、TLR4水平高于轻症组(P<0.05)。Pearson线性相关分析显示EOS患儿血清Cyt-C、HMGB1、TLR4与PCT、hs-CRP、IL-6、TBIL、ALT、AST、CK-MB、cTnI呈正相关,且血清Cyt-C与HMGB1、TLR4呈正相关(P<0.05)。血清Cyt-C、HMGB1、TLR4单独预测EOS的AUC分别为0.781、0.762、0.797,三者联合预测的AUC为0.954。结论EOS患儿血清Cyt-C、HMGB1、TLR4均增高,且Cyt-C与HMGB1、TLR4之间存在相关性,三者联合对EOS具有较高的预测价值。
Objective To explore the predictive value of combined detection of serum cytochrome C(Cyt-C),high mobility group protein B1(HMGB1),and Toll-like receptor 4(TLR4)on neonatal early-onset sepsis(EOS).Methods 97 children with EOS treated in our hospital from April,2019 to April,2021 were selected as the EOS group,while 80 newborns with simple bacterial infection and 82 healthy newborns delivered in our hospital during the same period were selected as the bacterial infection group and control group respectively.Serum levels of Cyt-C,HMGB1,TLR4,procalcitonin(PCT),high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),total bilirubin(TBIL),as well as alanine aminotransferase(ALT),aspartate aminotransferase(AST),creatine kinase-MB(CK-MB),and cardiac troponin I(cTnI)were compared among the three groups.Pearson linear correlation was applied to analyze the correlation between serum Cyt-C,HMGB1,TLR4 levels and inflammatory factors,liver function,and myocardial damage indicators,as well as the correlation between Cyt-C and HMGB1 and TLR4.The receiver operating characteristic curve(ROC)was drawn to calculate the area under the curve(AUC)of the three predictions of EOS.Results Levels of serum Cyt-C,HMGB1,TLR4,PCT,hs-CRP,IL-6,TBIL,ALT,AST,CK-MB and cTnI in the EOS group were higher than those in the bacterial infection group and control group(P<0.05).Levels of serum Cyt-C,HMGB1 and TLR4 in the severe group were higher than those in the mild group(P<0.05).Pearson linear correlation analysis showed that serum Cyt-C,HMGB1,TLR4 and PCT,hs-CRP,IL-6,TBIL,ALT,AST,CK-MB,cTnI in children with EOS were positively correlated.Serum Cyt-C in children with EOS was positively correlated with HMGB1 and TLR4(P<0.05).Serum Cyt-C,HMGB1,and TLR4 alone predicting EOS had the AUCs of 0.781,0.762,and 0.797,respectively,and the combined prediction AUC for these three was 0.954.Conclusion Serum Cyt-C,HMGB1 and TLR4 in children with EOS are all increased,and there is a correlation between Cyt-C and HMGB1 and TLR4.The combination of the three ma
