成人癫痫患者血清miR-146a、miR-125a、TLR4水平及临床意义

摘要 目的 探讨成人癫痫患者血清微小核糖核酸(miR)-146a、miR-125a、Toll样受体4(TLR4)水平及其与疾病严重程度的关系。方法 选取2020年8月至2023年8月新疆维吾尔自治区人民医院收治的69例癫痫患者作为癫痫组,另选取同期体检的69例健康人群作为对照组。采用实时荧光定量聚合酶链反应检测入组对象血清miR-146a、miR-125a水平,采用酶联免疫吸附试验检测血清TLR4、白细胞介素(IL)-1β、IL-2和肿瘤坏死因子-α(TNF-α)水平。根据脑电图检测结果,将癫痫患者分为轻度组、中度组和重度组,并比较3组患者血清miR-146a、miR-125a、TLR4、IL-1β、IL-2和TNF-α水平。采用Pearson相关分析癫痫患者血清miR-146a、miR-125a、TLR4水平与IL-1β、IL-2、TNF-α水平的关系,采用Spearman相关分析癫痫患者血清miR-146a、miR-125a、TLR4水平与疾病严重程度的关系。结果 癫痫组血清miR-146a、miR-125a水平低于对照组,TLR4、IL-1β、IL-2和TNF-α水平高于对照组,差异均有统计学意义(P<0.05)。脑电图检测结果显示,轻度组、中度组及重度组分别有23、25和21例,3组患者血清miR-146a、miR-125a、TLR4、IL-1β、IL-2和TNF-α水平比较,差异均有统计学意义(P<0.05);且血清miR-146a、miR-125a水平均为轻度组>中度组>重度组,血清TLR4、IL-1β、IL-2和TNF-α水平均为轻度组<中度组<重度组,任意两组间比较,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,癫痫患者血清miR-146a和miR-125a水平与IL-1β、IL-2、TNF-α水平呈负相关(P<0.05),TLR4水平与IL-1β、IL-2、TNF-α水平呈正相关(P<0.05)。Spearman相关分析结果显示,癫痫患者血清miR-146a和miR-125a水平与疾病严重程度呈负相关(P<0.05),TLR4水平与疾病严重程度呈正相关(P<0.05)。结论 成人癫痫患者血清miR-146a、miR-125a表达下调,TLR4表达上调,且三者与IL-1β、IL-2和TNF-α水平相关,可以评估癫痫患者疾病严重程度。 Objective To explore levels of microRNA(miR)-146a,miR-125a and Toll-like receptor 4(TLR4)in peripheral blood and their relationship with disease severity in adult patients with epilepsy.Methods A total of 69 patients admitted to the People′s Hospital of Xinjiang Uygur Autonomous Region from August 2020 to August 2023 were selected as the epilepsy group,and another 69 healthy people who had a physical examination during the same period were selected as the control group.Real-time fluorescence quantitative polymerase chain reaction was used to measure serum miR-146a and miR-125a levels,and enzyme-linked immunosorbent assay was used to detect serum TLR4,interleukin(IL)-18,IL-2 and tumor necrosis factor-a(TNF-a)levels in all research objects.Based on the results of EEG detection,epilepsy patients were divided into mild group,moderate group and severe group,and miR-146a,miR-125a,TLR4,IL-1β,IL-2 and TNF-αlevels were compared among the three groups.Pearson correlation was used to analyze the relationship between miR-146a,miR-125a and TLR4 levels and IL-1β,IL-2,TNF-αlevels,and Spearman correlation was used to analyze the relationship between miR-146a,miR-125a and TLR4 levels and severity of disease.Results The serum miR-146a and miR-125a levels in the epilepsy group were lower than those in the control group,and the levels of TLR4,IL-1β,IL-2 and TNF-αwere higher than those in the control group,with statistically significant differences(P<0.05).The results of EEG showed that there were 23,25 and 21 cases in the mild group,moderate group and severe group,respectively,and the levels of serum miR-146a,miR-125a,TLR4,IL-1β,IL-2 and TNF-αamong the three groups had statistically significant differences(P<0.05);the levels of miR-146a and miR-125a in the mild group>the moderate group>severe group,while the levels of TLR4,IL-1β,IL-2,and TNF-αin the mild group<moderate group<severe group,and comparisons between any two groups had statistically significant differences(P<0.05).Pearson correlation analysis showed that miR-1
